1470322838 2016-08-02 Production Wells No Violations I conducted a status check inspection of this well with Oil & Gas Inspector Ashley Raff (MRI 2) per the request of Norm Wengerd (CEO Gentle Brook) whose company is interested in purchasing the property which the well is located on. The well is a swab well with a small separator, a concrete in ground brine tank and it provides house gas to the residence on the property. The well, according to an involved realtor, is owned solely by the property owner. This conflicts with the RBDMS record, which shows the well owned by the Randell Oil Company. The well, according the RBDMS production records, is equipped with two tanks with a total capacity of 150 barrels, but no tank battery could be located for this well. Since there are discrepancies between what is reflected in RBDMS and what is on the ground I checked the Stark County Auditors web site for the Cosima #1 well information and found it is located in Osnaburg Township in Stark County on Georgetown St. approximately 1/10 mile west of Beck Ave. on the left side. I called the Randell Oil Company and spoke with Michelle. She advised me the Comsina #1 well located in Osnaburg Township is actually Comsia # 1 (34-151-21650-0000) which has 1060 listed as the permit number on the tank battery sign which is incorrect. She did not recall Randell Oil ever owning well (34-151-21060-0000) which is located on the Kurtz property is Lake township.

I met with Rick Anderson (Northeast District Supervisor) and Ashley Raff (Mineral Resource Inspector) on August 4, 2016 at the Division of Oil and Gas Uniontown office. After reviewing well maps and gathered information it was determined well # 34-151-21060-0000 is an unregistered domestic well which needs to be registered. I called Randell Oil Company and was informed the sign will be changed with the correct information at the Comsia #1 well, and the Columbus office of Oil & Gas will be contacted to correct the data associated with both wells.
