1386105366 2013-12-03 Production Wells No Violations I was onsite to inspect this well. The wellhead and stock tank are overgrown with vegetation. The stock tank has many seeps. The well has no SPCC or signage. This well has been inspected in the past and is a possible candidate for orphan and idle wells. It has open conductor casing with production casing sticking out. No calipers were present to measure the casing sizes at this time. It has a small gas leak on the wellhead and it is rusting out. This well will be referred to the orphan well program.
1388676002 2013-12-03 Production Wells No Violations I was on location to perform an inspection of this well. The wellhead and stock tank are overgrown with vegetation. The stock tank has many seeps but there are no major spills or oil on the ground in the area. There is no SPCC dike for this well. The tank battery contains a 100 barrel production tank. The well head appears to be a plunger lift well with a flow line to the tank. There is open drive pipe around the production casing at the surface. No calipers were present to measure the casing sizes at this time. A valve on the wellhead is hissing with a small leak on it. There is no signage on the well and this well is currently no in compliance. Inspector Claus issued a compliance notice on 07/27/2004 for contamination caused by this well. The compliance notice was cleared on 04/06/2007 and stated that this well is currently idle and orphan. A follow-up with the Idle and Orphan program will be made to check the status of this well being entered into the program.
1483101178 2016-12-29 Production Wells No Violations I conducted a status check inspection on this well. The well and tank are overgrown with vegetation. The well last reported production was in 1993 for 1902 MCF of gas. There is a small gas leak at the well and the tank has seeps. The well is not legibly identified. It has open conductor casing with production casing sticking out. A Chiefs Order # 2006-19 was issued on 01-30-2006 and the bond was forfeited. Within the findings of the Chiefs Order it was stated the division has been unable to locate any responsible party for the above referenced well. I met with Heidi Scott (MRI 3 - Orphan Well Program) for help reviewing records in the Stark County Recorders Office. Two documents were located in the records of the Stark County Recorders Office transferring an undivided half interest in this well. The documents are a Certificate of Transfer located in Vol. 215 Pg. 827 and a Quit-Claim deed found in Vol. 1699 Pgs. 615-618.
