-1292385780 2003-4-29 0:00: PB No Violations
1292385780 2003-04-29 Plug / Plug Back No Violations Located the plugged and abandoned well, Homer #1, permit #0635. The 7 dia. steel casing is visible at the ground surface. No evidence of leakage was observed. Took a GPS reading on the exposed casing: N 40.78125,W 081.36887. Reviewed the P/A information. The Homer #1 was properly plugged and abandoned on 10-11-1971.

Attempted to field locate well #60041; was unable to locate any physical evidence of the well. Contacted Steve Opritza in Columbus. There are no records of well #60041 and well #60041 does not appear on the historic National Fuel Gas maps. This well probably never existed and is considered a bad data point.

5-2-03. Contacted Trish Bishop of LJB Consulting (937-259-5047) in Dayton regarding the findings. Fax # 937-259-5100.
