| 0864444279 | 2011-12-01 | Production Wells | No Violations | I received a complaint call on 11-21-11 from Rachael Cook (919-475-1741) and Mark Cook (330-866-9375) regarding a possible idle well on their family farm located at 8395 Maplehurst Ave., East Sparta, OH 44626. I inspected the site on 12-1-2011. The well was drilled in 1942 and is currently a swab well. There is a new pressure gauge on the casing that is reading 50 psi. There were no leaks or contamination observed at the wellhead or at the swab tank. An old identification sign is posted on the swab tank. There is a 2 diameter flow line plumbed into a drip tank that goes into a sales line. A Gatherco pipeline marker, a small drip tank, and a house gas meter ( GPS 40.68322, 81.36334) are located 60 south of the wellhead. There was no production reported for 2009 and 7 MCF was reported for 2010. The well is currently owned by M & R Investments of Ohio. Based on my inspection, there are no violations at this well and the well is capable of production. |

The well was transferred to new ownership on 05-0302017 and the new owner has scheduled a meeting 07-10-17 with the division of Oil & Gas. This is a swab well with a swab tank. There is a domestic gas connection at the well. The well is not legibly identified. The well last produced 46 MCF of gas in 2012. I met with the landowners Mr. & Mrs. Cook and they expressed concerns about the domestic gas they use to heat there house. I gave them my business card with the contact number for the new owner. |

There is no tank battery for this well. The well is located more than 200ft. away from any private inhabited dwelling house not located on the tract where the well is located. The well is more than 200 ft. from any public building. I called 330-866-9375 to speak with the landowner and no one answered. When I arrived on location, I knocked on the residence door no one answered so I left my business card with a note requesting a call back. I called 330-866-9375 on 02-12-2019, there was no answer and I have not received a return call as requested on my business card. |
