1076006250 2004-02-04 Production Wells No Violations -On site for a status check of well listed on get activities list. Where GPS reading indicated the well was located, narrow orange plastic pipe sticking from the ground. Small approximate 10 by 10 wetland area with cattails growing around the area. Water was dark and orange in the wetland. Small of oil/gas noted in the area. No wellhead or equipment was noted. Supposed well location in low area of a farmfield near East Turtle Creek. Digital photos taken of the area.

-In discussion with Jeff Fry on 2/5/04, we may jointly visit the site with a metal detector to determine where the well is located. No production reported since 1993 when gas was reported. No information is showing that the permit has been plugged.

1077550570 2004-02-18 Production Wells Violations Noted -On site for status check on get activities list. Joint inspection with Jeff Fry and Mike Mitchell. Site was inspected on 2/4/04 and casing was not located by gas smell detected. Small wetland was located nearby with cattails growing around a dark colored wetland area. After digging under weeds on 2/18/04, well casing was located. Well casing had been run over in the past and bent to the ground. Orange plastic pipe is present showing the location of the well. No gas production reported since 1993. Well incapable of production.
1080589355 2004-03-22 Production Wells Violations Noted -On site for follow-up inspection and mtg with John Firko, of Power Gas Marketing (acquired EDC wells in mid-1990s). Also met with Bob Huwer of Huwer Drilling. N.O.V. # 1077550570 issued 2/23/04 for the well being incapble of production. At some point in the past, gas regulator has been runover/broken off. Some gas is venting into the air. Well located while on site near small wetland area. 10 conductor casing down to 50 w/ 7 surface casing to 289. Total depth to 1270 (drillers) &1272 (loggers). Well drilled into the Trenton Formation. Well not labelled. John indicated he was going to have Bob plug the leak to start. John to investigate if company will put the well back into production and if not, will plug the well. Notified John that I would give an extension on plugging the well.
1197559642 2007-12-12 Plug / Plug Back No Violations -Cancelled N.O.V. # 1080589355 issued 2/23/04 for well not in production & not labeled. Operator ordered to ID well & either plug or produce. Temple Oil contracted by well owner Power Gas Maketing-North Coast Energy to plug well. 50 of 10.75 surface casing & 290 of 7 left in hole but the upper 30 cut off. Cemenco Services plugged the well w/ tremie pipe used to pump Class A cement w/ 4% bentonite gel additive. Bottom plug from 1275-1042 w/ 35 sacks of cement; middle plug from 390-190 w/ 30 sacks; & top plug from 100 to 30 below surface w/ 17 sacks of cement. Freshwater used as spacer w/ 2.75 bbls btwn bottom & middle plug & 0.75 bbls btwn middle & upper plug. Bridge plug used beneath upper plug. Well is artesian w/ water outside the 10.75 casing w/ some organics & is flowing to nearby creek in ditch. Well to be cut off to 30 below, ditch to be left for the artesian water to flow. Photos taken. Final restoration inspection on 12/20/07.
1198094555 2007-12-18 Plug / Plug Back No Violations -On site for status check inspection of well plugged on 12/12/07. Since the date plugged, field conditions including muddy fields on 12/12/07 and 3-4 snow on ground on 12/18/07 has prevented plugging contractor from doing final restoration. Well is an artesian w/ water flowing to creek to the north in a main ditch w/ an auxillary ditch to the west. Some organics seen when well was plugged on 12/12/07 but no oil seen in creek on 12/18/07. Photos taken of the site. Talked to Dana Blue later and requested he place plastic tile, cover in gravel, and fill soil back over the ditch to allow water to drain to creek. Plugger to also grade out the ruts in the access road when weather permits. 3-4 bales of hay stored on site for use in restoration.
1199827936 2008-01-08 Plug / Plug Back No Violations -On site for restoration work on the well. Well plugged on 12/12/07 by Temple Oil & Gas. Wet field conditions and snow has prevented Temple Oil from doing the restoration work since well plugged. Well is an artesian well with water flowing to the creek to the north in main ditch w/ a secondary ditch. Operator deepened the main channel dug during plugging, placed plastic tile pile in a sleeve-sock and began to cover up the tile. Tile drains to unnamed creek to north. Field conditions were too wet around the former well location to finish the work. Operator to back blade the access road and come back in drier weather to finish the work. Photos taken of the prelim work. Request Dana Blue contact me when he comes back up to finish the restoration.
1212501931 2008-06-02 Plug / Plug Back No Violations -On site for additional restoration work by Temple Oil & Gas. Temple plugged the well on 12/12/07. Wet field conditions & snow prevented Temple from doing the well at the time. Well is an artesian well with water flowing to the creek to the north. Temple did preliminary work including digging channel & placing plastic tubing to try to drain area until the area dried out. The lease road was back bladed, seeded, & mulched at that time. On site for additional work. A deeper trench was dug to the creek, with water drained to the creek, & new plastic tubbing with sleeve placed in trench, 57s added above the tubbing, and the trench filled back in. The area around the well was graded up and later seeded. Will look at the site on 6/3/08 to see work completed after leaving the site. Will monitor the site to see that grass is established. Also met with landowners son Greg Bensman while on site. Photos taken.
1212667695 2008-06-04 Plug / Plug Back No Violations -On site for follow-up to the restoration work done on 6/2/08 after I left the site. The area surrounding the former well has been graded even though a portion is very soft. The access road to the well site is vegetated. Due to heavy rain on 6/3 & 4/08, a portion of the graded up area is under water as is the soybean field. Photos taken of the restoration area and the lease road into the well. After leaving the site on 6/2/08, Temple Oil & Gas seeded the area. Will continue to monitor to see that grass becomes established. The well was originally plugged on 12/12/07. Wet field conditions around the well and in the area in general prevented Temple from doing the final restoration work at that time. Some temporary work was done including digging channel, installing plastic drain pipe to the unnamed creek to the north. The lease road was back bladed, seeded, and mulched. Temple did the final restoration work on 6/2/08 including digging a deeper channel, installing new drainage tile with a sleeve over it, placing 57s over the drainage pipe, and then graded & seeded.
1216224811 2008-07-15 Plug / Plug Back No Violations -On site for follow-up restoration inspection per request by Dana Blue of Temple Oil on 7/14/08. The additional restoration work was done on 6/2/08. Photos taken of the area where the well was plugged. Well initally plugged on 12/12/07 and some prelim restoration work done. O/G well caused water to come to the surface (artesian) and prelim restoration work done in 12/07. Ground was too wet at that time so additional work done on 6/2/08 including installing new plastic tile, new sleeve over the tile, covering the tile with aggregate (57s), covering and seeding the area. Water is draining to the unnamed creek. Vegetation is beginning to become established where the well and the buried tile is located. There are 2 low areas on each side of the former well location. Voice mail left w/ Dana Blue on 7/16/08 and requested he contact the landowner to see what landowner wanted. If okay with landowner, will approve final restoration. If not, place more material to fill the two low areas.
1217944420 2008-08-04 Final Restoration No Violations -On site for final restoration inspection. Phone mtg-call from Dana Blue of Temple Oil while on site. Additional restoration work was done on 6/2/08. Well initially plugged 12/12/07 & some prelim restoration done. O/G well created artesian well. Ground too wet in 12/07 to do the work. Work on 6/2/08 included installing new tile, new sleeve over tile, covering tile w/ 57s, covering & seeding the area. Water is draining thru tile to creek. The 2 low areas have dried up. Since last inspection on 7/15/08, Dana contacted landowner & he is happy w/ 6/2/08 work. Some flooding on creek took out some beans and vegetation around well. Area to be tilled & farmed in 2009. Photos taken. Will approve final restoration. NOV 1077550570 was issued 2/18/04 for well incapable of production. NOV terminated 8/5/08.
