U.S. Energy ran 14 joints of new, 26lb/ft. casing to a depth of 587 measured from the kelly bushing. Petroset Cementing cemented the casing with 53 barrels (250 sacks) of Class A cement slurry. 11 barrels of good cement circulated to surface during the job. 0475824863 2019-02-11 Completion Testing No Violations Status check inspection of this recently drilled, two (2) well pad. My inspection found that 14-500 barrel frac tanks have been spotted on the west side of the location. Moore Well Service is currently filling the northern pit with water from the Sekel pad. Completion operations are scheduled to begin on 2/18/19. The wellheads are secure and surrounded by jersey barriers. No violations.
0755838899 2018-09-28 Drill / Deepen / Reopen No Violations Status check inspection of the horizontal, Clinton Sandstone well pad. The pad is complete, the cellars are set, and the pits have been excavated. This area is a historic coal surface mining area and the entire location is fill below the top soil. The pits have large chunks of fill rock throughout the excavated area. I have spoken with U.S. Energy concerning the preparation of the pits prior to installing the liners. The side walls and base will be covered with straw/hay and road fabric will be installed on top of this layer. They have indicated that a geo-textile material will also be emplaced between the fabric and liner. There are no violations at this time.
0854378181 2018-12-21 Drill / Deepen / Reopen No Violations Status check inspection of U.S Energy Rig #9. The rig has drilled a 6.175 hole out from under the 7 intermediate casing and landed the curve at a measured depth of 5114. They have tripped the drill string out of the hole and closed the blow out preventer. They are currently moving the blow line and setting up for fluid drilling. The pits are in good condition and have plenty of freeboard. They are going to secure the location and shut down for the holiday. They will resume operations on 12/26/18. I found no violations at this time.
0908669782 2019-01-14 Drill / Deepen / Reopen No Violations Status check inspection of U.S. Energy Rig #9. They are currently drilling a 6.175 hole at a total measured depth of 6910 (KB +10). Their current rate of penetration is between 6ft./hour & 11ft./hour. The azimuth is 301.5 degrees and the drilling angle is 90.6 degrees. Expected total depth should be reached in 8-10 hours and cementing of the 4.5 production casing is estimated in the next 24-36 hours. The pits are in good condition and have plenty of freeboard. They have just placed heaters on the heat tubes for the cement mix water. There were no violations at this time.
0920716389 2019-03-25 Completion Testing No Violations Status check inspection of completion operations at this horizontal, Clinton Sandstone well. Moore Well Services (MWS) was rigged up over the hole and have drilled out all of the kill plugs. They are currently shut down and fixing the swivel. They pulled the tubing back into the vertical section of the well and will continue cleaning up the well tomorrow. No violations.
1148969997 2019-03-18 Completion Testing No Violations I was on location to observe hydraulic fracturing operations conducted by Sure Fire Wireline (SFW) and Producers Services Corp. (PSC). Stage #2 has just been completed and SFW rigged up the lubricator and gun. They pumped the gun down hole and set the kill plug at a depth of 6720. SFW perforated Stage #3 from 6640-6700 with 50 shots and came out of the hole. PSC rigged up their iron, tested the lines, and broke the formation. The stage was treated with 500 gallons of 20% HCl, 175 sacks of 20/40 sand, and 1000 barrels of water. There were no violations at this time.
1182002850 2018-12-12 Casing String Witnessed No Violations I was on location to witness the cementing of the 7 intermediate casing string. U.S. Energy ran 95 joints of casing to a depth of 4246 measured from the kelly bushing. Petroset rigged up to the casing, conditioned the hole, and cemented the casing with 84 barrels (400 sacks) of Class A cement slurry. Good circulation was maintained throughout the job. 1346337854 2019-03-26 Completion Testing No Violations Status check inspection of completion operations at this horizontal, Clinton Sandstone well. Moore Well Services (MWS) was rigged up over the hole and have laid down all of the flow back tubing. They are currently rigging up to swab the 4-1/2 production casing. The pits are mostly empty and should be solidified and closed next week. Four (4) frac tanks are being moved to the Sekel Pad and the pumping rods and separator are being delivered. There were no violations at this time.

There were no violations at this time.

They are currently drilling on air at a depth of 245 at a rate of 78/hour. I inspected the 13.375 casing on the racks and it is new, 36lb/ft, and there are 8 full joints (Range-3). There are three (3) bow spring centralizers outside the tool shack. The pits are in good condition and the cuttings are being misted to the blow pit. No violations.
1544538875 2018-12-11 Drill / Deepen / Reopen No Violations Status check inspection of U.S. Energy Rig #9 drilling operations. The rig is currently drilling an 8.5 hole at a depth true vertical depth of 3029 measured from the kelly bushing (KB +10). They are currently drilling on air and misting at a rate of 72/hour. The pits are in good condition and have plenty of freeboard. I inspected the 7 casing on the racks and it is new 23lb/ft and 20lb/ft, J-55, Range-3, stamped API, WTC, 5CT. I found no violations at this time.

My inspection found U.S. Energy Rig #9 drilling the 6.175 lateral at a measured depth of 5840. The current azimuth is 373 degrees and the angle is 91.45 degrees. The current rate of penetration is 5ft./hour at a true vertical depth of 4830 in the White Clinton Sand. The pits are in good condition and have plenty of freeboard. No violations.

The rig has currently drilled a 6.175 hole to a total measured depth of 6,195 and have just begun to trip out of the hole to change out the drill bit. The pits are in good condition and have plenty of freeboard. The lease road and drilling pad are in good condition. The tool pusher expects to be back on bottom and drilling in 12-14 hours. The permit is posted in the dog house as required. No violations.
1547825514 2019-01-15 Casing String Witnessed No Violations I was on location at 9:32pm to witness the cementing of the 4.5 production casing string. Petroset Cementing (PCS) was on location and U.S. Energy Rig #9 was running the remaining 30 joints of casing into the hole. The casing was landed at a measured depth of 6962 and packed into the 7 wellhead. PCS rigged up, conditioned the hole, and cemented the casing with 84 barrels of Class A cement slurry. Good circulation was maintained throughout the job and the plug down was at 12:30am. See casing ticket for more details on this job. No violations.
1548268604 2019-01-22 Drill / Deepen / Reopen No Violations Status check inspection of mobilization operations following completion of drilling and cementing activities on 1/16/19. U.S. Energy personnel are currently strapping down three (3) loads for transport to the Scrocca location (PN#24567) in Ashtabula County. The rig sub-structure (2 loads) and the mud pump are ready to move. The derrick on the Speed Star has been lowered and the blow and flow lines have been disassembled. The tool pusher expects to be ready to spud the week of 1/28/19. The pits are in good condition and have plenty of freeboard. No violations.

1553179495 2019-03-21 Completion Testing No Violations I was on location to check the status of completion operations by Sure Fire Wireline (SFW) and Producers Services Corp. (PSC). Harry Mills was the U.S. Energy Company man on location. Harry told me that Stage #5 sanded off early in the job and that Moore Well Services had to rig up and run tubing to perforate Stage #6 and was fractured with 900 barrels of water and no sand. They have just completed Stage #7 with 175 sacks of sand and 800 barrels of water. Break down pressure was 6900psi and treated at 2730psi after the sand cut the perforations. SFW ran in the hole, set a kill plug at 6320 and perforated the casing with 50 shots from 6240-6300. They came out of the hole and PSC rigged up their iron to treat Stage #8. No violations.

The pits are in good condition and have plenty of freeboard. I spoke with the tool pusher and they are planning to spud the 8.75 intermediate hole on Monday (12/10/18). No violations.
2124447028 2019-02-04 Preliminary Restoration No Violations Status check inspection of this drilled, horizontal, Clinton Sandstone well. Preliminary Restoration is due by June 2019 for this multi-well pad. My inspection today found that all drilling equipment has been removed from the location. The wellhead is surrounded by jersey barriers and the 20 X 13.375 and 13.375 X 9.625 annuli are open and not equipped with casing heads. The 9.625 X 7 and 7 X 4.5 annuli are equipped with nipples and two (2) inch ball valves on the casing head ports and are open. The 4.5 production casing is equipped with a 4 X 2 reducer with a closed ball valve, bull plug and needle valve. The drilling pits remain open and I have extended the required time frame for closure of the pits until fracturing operations have been completed, as they will be utilized for freshwater storage at that time. The pits are currently in good condition and have plenty of freeboard. The location and lease road are stable and the gate at the location entrance is locked and secure. No violations.
2138261480 2018-11-28 Drill / Deepen / Reopen No Violations Status check inspection of drilling operations of U.S. Energy Rig #9. Rig personnel are currently disconnecting electric lines and flow lines in preparation to skid the rig to this well location and rig up. I retrieved the 4.5 production casing cement job log from the doghouse and inspected the drilling pits. The pits were in good condition and have plenty of freeboard. There were no violations at this time.
