0077786214 2018-11-16 Surface Facility Construction No Violations Status check inspection of surface production equipment construction and installation that will service this well and the Sekel Unit #1H (PN# 24473). Moore Well Services has spotted and plumbed together three (3) 210 barrel tanks on the north side of the location. They are currently moving the fire-heated, gas production unit into position east of the tank battery. The tank, wellhead, and separator spacing meets the requirements outlined in ORC 1501:9-9-05. The wellhead has been completed with 2.375 tubing and rods and is ready for the pumping unit to be installed (not currently on location). The pad and lease road are snow covered and in stable condition. No violations.

Moore Well Services is continuing to mill on an unknown obstruction inside the 4.5 production casing. They made a bit trip yesterday and are currently drilling/milling with a 3.375 tri-cone bit. They are currently at a depth of 5580 and have made between 80-100 of progress since Monday (10/1/18). Returns are yielding steel cuttings, pieces of rust, and brass from the bolts inside the kill plugs. U.S. Energy is currently loading the 3 drill pipe and racks and moving them to the Frame pad. No violations.

0276666776 2018-10-02 Completion Testing No Violations Status check inspection of completion operations on this horizontal, Clinton Sandstone well. My inspection found Moore Well Services rigged up and milling a steel obstruction encountered between kill plugs #7 & #8. They are milling with a 3.375 concave, carbide mill. The cuttings captured on the magnet from the return line did not appear to be casing or a hardened steel. They are currently at a depth of 5500. They increased the mud weight with flocculant and brought around more cuttings with an odor of cement. A pH strip was run on the returns and registered between 9 & 10. All indications were that the cuttings were generated from the perforations and not from drilling out of the 4.5 production casing. They will continue to mill up the obstruction to the top of plug #7. No violations.
0281432933 2018-05-14 Drill / Deepen / Reopen No Violations Status check inspection of pad construction for this horizontal, Clinton Sandstone well. My inspection found the lease road and apron completed and stable. I met with Dave (site construction) and he said that the rain has prevented any further construction of the pad. I inspected the pad and it is approximately 200 X 100 and the southwest quarter of the pad has been stoned. The cellars for this well and the 1H (PN#24473) have been installed and the bottoms have been sealed with cement. I took GPS readings of the cellars (wellhead locations) and they agree with the submitted location plats. There are two (2) bull dozers, a trac-hoe, and a roller on location. I will check back later on construction progress. No violations.
0444873865 2018-07-12 Casing String Witnessed No Violations I was on location to witness the running and cementing of the 13.375 surface casing string. U.S. Energy Rig #9 ran six (6) joints of 36lb/ft., Range 3 casing into a 300 deep, 17.5 hole with a 15 pup joint on bottom and landed the pipe with a 10 joint at a depth of 267 measured from ground level. The casing was equipped with three (3) bow spring centralizers. Petroset Cementing cemented the casing with 63 barrels of Class A cement slurry and 20 barrels of cement circulated to surface. See casing ticket for more details on this job. No violations.
0620124098 2018-10-09 Completion Testing No Violations Status check inspection of completion (drill-out) operations. Moore Well Services is conducting the drill out of the kill plugs in this horizontal, Clinton Sandstone well. They are currently tripped out of the hole and have lost the three (3) cones from the 3.875 tri-cone bit. They are having discussions with U.S. Energy on how to proceed to retrieve the lost cones from the wellbore. They are debating whether to use a magnet or a junk basket. No other operations will be conducted today. No violations.
0777432057 2018-10-04 Completion Testing No Violations Status check inspection of completion operations. Moore Well Services was on location performing drill out operations. They are currently tripping out of the hole with a five (5) tooth mill to examine the bit. Examination of the bit pulled yesterday is indicating that they are drilling through the middle of the kill plugs and the grapples are not releasing from the casing walls. There plan is to trip back in with a tricone bit to see if the grapples are still in place. U.S. Energy is continuing to move the ancillary equipment for Rig #9 to the Frame Pad. No violations.

My inspection found that the electric pumping unit has been installed and is currently in operation. The valve on the 4.5 production casing port is closed and the sales line is currently being installed to the tie in point at the Sekel A #s 1 & 2 tank battery located 800 to the northeast. Cary Leshock (Operations Supervisor) is on location and adjusting the Baird valves for optimal pressure. Identification has been posted at the wellhead and at the tank battery. No violations.
0939725795 2018-07-16 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the 9.625 intermediate #1 string to cover the Berea Sandstone. U.S. Energy Rig #9 drilled a 12.25 hole to a depth of 522 measured from the kelly bushing (KB+10) and set 26lb/ft., casing at a depth of 507 (KB) with a float shoe on bottom and equipped with six (6) bow spring centralizers. Petroset Cementing Service (PCS) arrived on location and loaded the wiper plug in the cement head. They rigged up and established circulation and then conditioned the hole with 200lbs of Bentonite Gel and 40lbs of cottonseed hulls. They then mixed and pumped 52.5 barrels of Class A cement slurry (verified at 15.3lbs/gal.), dropped the plug, and displaced the plug to the shoe with 41 barrels of water. The plug landed at 200psi and the pressure was increased to 600psi and held for five (5) minutes with no leak off. The pressure was released, the float held, and the valves on the cement head were closed. 18 barrels of good cement circulated to the cellar during the job. Mix water pH was 7, water temperature was 95 degrees, and chlorides were less than 170ppm. No violations.

1204722496 2018-12-27 Preliminary Restoration No Violations Status check inspection for preliminary restoration of this horizontal, Clinton Sandstone well location. My inspection found the well is in production and is equipped with 2.375 tubing and rods, and operating with the aid of an electric powered pumping unit. Gauges indicate 250psi on tubing and 450psi on 4.5 production casing. A gauge indicates 100psi on 4.5X 7 annulus and it is equipped with a 250psi pop-off valve. The 7 X 13.375 annulus is plumbed to surface and open. The stuffing box is fitted with a pollution control device that is plumbed to a 55 gallon drum. This well produces through an indirect, fire-heated, horizontal separator and into three (3) 210 barrel tanks. All the surface production equipment is within secondary containment and is shared with the Sekel #1H (PN#24473). A 500 barrel frac tank has been spotted within secondary containment and is being used to transfer brine from the 210 barrel tanks for disposal. The required ID is posted at the tank battery and the wellhead is properly identified. There are currently 12-500 barrel frac tanks on location that were utilized for completion operations. The tanks have been emptied and will be moved to the Frame Pad for completion operations in the next month. The drilling pits remain open and contain cuttings and freshwater. I have spoken with U.S. Energy concerning the pits and have extended the time frame for closure until the water can be transferred to the Frame Pad for use in fracturing operations. The liners are intact and the pits have plenty of freeboard. Drilling was completed on this pad on 7/10/18 and preliminary restoration is due by 1/10/19. This time frame has been extended into the spring when a good seed bed can be established. No violations.

Dansco is currently setting the Lufkin pumping unit on the cement base of this well. The dike is being constructed around the tank battery at this time and contains three (3) 210 barrel tanks. Equipment spacing meets requirements. The frac/flow-back tanks are being emptied by Moore Well Services. There were no violations at this time.
1428855402 2018-11-26 Surface Facility Construction No Violations Status check inspection of production equipment installation. My inspection found the wellhead tied into the fire-heated, horizontal separator shared by the Sekel #1H (PN#24473). The dump line to the tank battery has been installed and there are three (3) shared, 210 barrel tanks. The dike around the tank battery is partially constructed. The electric powered pumping units have not been installed and the power supply has been set and the lines are trenched in. All equipment spacing meets the requirements of OAC 1501:9-9-05(A). There are currently 15-500 barrel frac tanks on location that were used for completion operations. Much of the ancillary equipment used for the drill out and flow-back remains on location. The drilling pits are still open and need to be emptied, solidified, and closed. I will speak with U.S. Energy about their time frame for getting the pits closed. There are currently no violations.

They did not work this weekend and they are still planning to kill the well and resume fishing operations when the well is laid down. All fluids are being flowed back and contained in a 500 barrel, steel tank. The location and lease road are in stable condition. No violations.

The drilling pits are in good condition and have plenty of freeboard. I found no violations at this time.

The crew is currently rigging down and preparing to mobilize equipment to the Wildman #s 1H & 2H in Trumbull County (PN#s 4107 & 4108). The wellhead is secured and the annular valves are open. The pits are in good condition and have plenty of freeboard. I found no violations at this time.
1535463357 2018-08-28 Completion Testing No Violations Status check inspection of this drilled, horizontal Clinton Sandstone well. My inspection found the rig crew moving the drilling equipment to the west side of location to make room for completion operations scheduled to begin next week. 14 frac tanks have been spotted along the north side of the pad on the south side of the drilling pits and are currently being filled with freshwater by Moore Well Services. Pit water (brine) is being pumped into half of the frac tanks and being treated with a biocide. The pits are in good condition and have plenty of freeboard and the wellheads are secured with jersey barriers. No violations.
1538145776 2018-09-28 Completion Testing No Violations Status check inspection of completion operations on this horizontal, Clinton Sandstone well. Moore Well Services is rigged up and tripping into the hole with tubing and a tri-cone drill bit to begin drilling out the kill plugs inside the 4.5 production casing. They are currently at a depth of 2500 and will reach the first plug at 4348. The location is in good condition, the #1H is secure, and the blow out preventer is rigged up. The pits are in good condition and have plenty of freeboard. No violations.

Moore Well Services was on location and tripping back into the 4.5 production casing with a 3.75, concave, five (5) blade mill. They were able to get through kill plug #8 with the previous mill and still had junk in the hole. They are going to continue to drill and mill until the hole is cleaned up. U.S. Energy personnel are on location and mobilizing Rig #9 and the ancillary drilling equipment to the Frame Pad (PN#s 24474 & 24475) on U.S 224. There were no violations at this time.
1539620610 2018-10-15 Completion Testing No Violations Status check inspection of completion operations. Moore Well Services is currently flowing the well back to a steel tank and the plan is to load the hole intermittently to kill the well so they can continue to fish metal debris out of the 4.5 production casing. They made three (3) trips with a magnet over the weekend and recovered metal cuttings and one (1) of the three (3) cones lost from the drill bit. They were unable to get the junk basket through the curve. They are going to continue to flow back to kill the well. No violations.

2027361007 2018-07-19 Drill / Deepen / Reopen No Violations Status check inspection of this horizontal, Clinton Sandstone, drilling operation. U.S. Energy Rig #9 is currently drilling an 8.75 open hole, on air, at a depth of 3900 at a rate of 150/hour. The casing is stacked out on the racks and there are 20 joints of 23lb/ft., 7 casing to be run on bottom with a shoe and 78 joints of 20lb/ft., 7 casing of which they plan to run 74 of the joints. All casing is stamped API 5CT. There are 19 bow spring centralizers stacked out beside the rig. The pits are in good condition and the cuttings are being misted in the blow line to keep the dust under control. I found no violations at this time.
2071092617 2019-02-11 Preliminary Restoration No Violations Status check inspection of this two (2) well pad. My inspection found that the frac tanks used during completion operations have been removed from location. Moore Well Services is on location and hauling water from the pits and transferring the water to the Frame pad for use in completion operations. I spoke with Harry (U.S. Energy) and the pits will be solidified and closed in the next two (2) weeks and Preliminary Restoration can be approved after this area is reclaimed and stabilized. No violations.

There are three (3) loads of top dressing on the north side of the pad. I spoke with Harry (U.S. Energy) and he told me they should have the pad finished and begin digging pits by the end of the week and start mobilizing the drilling equipment. No violations.
