0229242359 2014-01-10 Preliminary Restoration No Violations I was on location to check the status of the blow pit being solidified and closed. ChemTech was delivering the fourth load of dust to the pit and the excavator was mixing at the south end of the pit. Enervest was swabbing the 4.5 casing dry in preparation for perforating the well. All fluids were being swabbed to the lined working pit. Stallion was hauling loads of freshwater in and filling 3-500 barrel frac tanks on the north side of the location. No violations.
0446457298 2018-12-12 Production Wells No Violations Status check inspection of this Rose Run Sandstone well. My inspection found the well in operation and equipped with 2.375 tubing and rods and an electric powered pumping unit. This well produces to a diked tank battery on location and contains one (1) 210 barrel tank, a 50 barrel poly tank, and a horizontal separator. The chart at the separator indicates a differential production pressure of 30psi. The required ID is posted on the tank. No violations.

The lease road was stoned back to the location and excavators were clearing the site of trees, brush, and large blocks of sandstone from an old barn foundation. The location was dry and in good condition. No violations.
0548592814 2014-02-07 Completion Testing No Violations I was on location to inspect the completion testing of this Rose Run well. The well was currently shut in and rods and tubing have been spotted in the hole. There is 1150psi on the 4.5 casing and the gauge on the tubing is closed. There are 3-500 barrel tanks on the north side of the location. The middle tank was used for flowback and is 3/4ths full. The other tanks are empty. The location is in good order and I found no violations at this time.

0614751383 2014-02-20 Completion Testing No Violations I was on location to check the status of this recently completed Rose Run well. Enervest personnel were on location building the dike around the tank battery. An electric pumping unit had been spotted over the hole and was in operation. The well was being tested through a horizontal separator to a new, 210 barrel tank and a 50 barrel poly tank in the tank battery . The gas was being sold through a steel line running north along the ground surface and tied into a preexisting sales line halfway up the lease road. The power for the pumping unit was inside a plastic conduit on the ground and run from the electric drop at the Miller #1 (PN#22564) to the north. The location is good condition. No violations.
1006381474 2013-12-18 Drill / Deepen / Reopen No Violations I performed and inspection on the above location. They were drilling in the Queenston at a depth of 5445 with a PDC bit. Permits were posted. Fire extinguishers are on location. I instructed the drilling Forman to have the drilling pits inspected quite often due to the amount of rain that is in the weather forecast for the next few days. No violations were noted at this time.
1095541260 2014-09-26 Preliminary Restoration No Violations I was on location to inspect for preliminary restoration at this well location. The location has been graded, seeded with wheat, and the seed bed is beginning to establish itself. There is no drilling equipment on location and the pits were solidified and closed in January. Preliminary restoration is approved. No violations.
1268988520 2014-03-10 Production Wells No Violations I conducted a status check inspection of this recently completed Rose Run well. The well is currently producing through a horizontal separator to a 210 barrel tank and a 50 barrel poly tank which are contained within the diked battery. There was 630psi on the wellhead during the inspection. Enervest arrived on location during my inspection to haul production water to their disposal well (#23343) in Nelson Township. The 80 barrel vac-truck was properly identified with Brine in large letters and the proper UIC #386. No violations.
1304114290 2013-12-11 Drill / Deepen / Reopen No Violations I was on location to witness the testing of the blowout preventor (BOP) and verify the integrity of the 8.625 intermediate casing string. PEP Rig #10 placed 500psi on the wellbore and closed the annular BOP. A hammer union on the kelly supply line was spitting drilling mud and needed to be tightened to conduct the test. The connection was fixed and the rig charged the wellbore to 475psi and closed the BOP. After 30 minutes, there was no change in the pressure reading on the Pason Gauge. The BOP test passed and casing integrity has been verified in accordance with OAC 1501: 9-01-08 (D)(3)(b)(ii). No violations.
1341116899 2013-12-02 Drill / Deepen / Reopen No Violations I was on location with Enervest and PEP drilling to check on drilling pit construction. Water was encountered 10-12 below grade and it was decided to make the pits shallower and wider to avoid the groundwater. The test pit will be pumped dry, backfilled, and sprayed with a bentonite slurry to seal out the groundwater prior to liner installation. No violations.
1380143313 2015-11-09 Production Wells No Violations I received a call from Kyle Ahbel (Enervest) on 11/8/15 at 10:34am to report the overflow of a 210 barrel tank on this location. He told me the spill was contained and was mostly brine with a skim of oil. He also told me the water truck driver had vacuumed most of the spill out of the containment dike. I met with Kyle this morning to assess the spill. Approximately 2-3 barrels had overflowed the tank. I tested the standing water near the tank and maxed out a high range Quantab indicating high chlorides. I also tested the standing water on the north side of the battery, in front of the separator, and found normal levels of chlorides. There was oil staining on the gravel around the tank and it was decided to steam clean the tank battery to remediate the spill. Kyle is going to schedule a steamer truck for this week. I found no brine or oil outside of the containment dike. I will oversee clean-up operations.
1388758436 2013-12-09 Casing String Witnessed No Violations I was on location to witness the cementing of the 11.75 surface casing string. PEP Rig #10 drilled a 17.5 hole to 120, pulled the drill string and tripped in with a 14.75 tricone bit. They TDd the hole at 478 and ran 11 joints of 11.75 casing to a depth of 463 and a 10 landing joint. Universal rigged up to the cement head, tested the lines, established circulation, and pumped 15 barrels of gel and 5 barrels of water spacer. They then mixed and pumped 64 barrels of Type 1 cement slurry and displaced the slurry to 443 with 55 barrels of water. 18 barrels of good cement circulated and held at the surface and the cement head was shut in. 1389026916 2014-01-06 Drill / Deepen / Reopen No Violations I was on location to check the status of the drilling pits following the completion of the well. The drilling rig and associated equipment have been moved from location. Three frac tanks remain on location as well as a bulldozer. Stallion was on location checking the tanks which had been flushed out and emptied. The drilling pits were still open and nearly empty. The remaining water in the pits is frozen and will need to thaw out before hauling the rest off of location for disposal. The liners are intact and the pits have plenty of freeboard. I spoke with Enervest and they hope to get the pits solidified and closed by the end of the week. No violations.
1389209875 2014-01-08 Drill / Deepen / Reopen No Violations I was on location to check the status of the site. Sutton Pump and Supply was installing the frac valve on the wellhead. The pits remain open, are fenced, and what liquid remains in them is frozen. Enervest personnel are on location to plug the mouse and rat holes with Quickrete. They are hoping to close the pits by the end of the week. No violations.
1391108352 2014-01-30 Completion Testing No Violations I was on location to inspect flow back and completion testing operations on this Rose Run Well. Chipcos service rig was over the hole and they will be rigging down and moving off location tomorrow. Enervest was going to rig up and swab the hole to steel tanks this afternoon. No violations.
1468143061 2013-12-10 Casing String Witnessed No Violations I was on location to witness the cementing of the 8.626 intermediate casing string. Pep Rig #10 drilled an 11 hole to a depth 1137, ran 23 joints of 8.625 casing to a depth of 1024, and rigged up the cement head. Universal racked in, tested the lines, established circulation, and pumped 15 barrels of gel spacer. They then mixed and pumped 78 barrels of cement slurry and displaced the slurry to 1006 with 64.5 barrels of water. 24 barrels of good cement returned and held at the surface and the head was shut in. 1549866577 2015-11-17 Production Wells No Violations I was on location to oversee the steam cleaning of the tank battery floor. Scott Miller Contracting was on location with Barton Oilfield Services steamer truck to conduct operations. A sump was dug in the tank battery and the affected gravel on the floor of the tank battery was steam cleaned with 35 barrels of hot freshwater. The water was recovered in the sump and trash pumped into the 35 barrel poly tank in the battery. The recovered water will be hauled to a disposal well. The affected area of the tank battery has been remediated. No violations.
1617398441 2014-01-14 Preliminary Restoration No Violations I was on location to inspect the solidification and closing of the working pit. The contractor had mixed in 3 loads of dust. A chunk of mud thrown into the slurry revealed that it was in need of another load of dust to fully set. They will have another load delivered this afternoon. No violations.
1682936684 2014-01-21 Completion Testing No Violations I was on location to check the status of the well following completion operations on 1/20/14. Chipco was rigged up over the hole and running in with tubing and a sand pump to clean out the bottom of the well. I spoke with Jeff Foley (Drilling Supervisor) and he told me that they had flowed back 400 barrels of water in the last 24 hours. The flow back was going to a steel tank on the north side of location. During flow back, the hole filled up with 200 of proppant (sand) and they were preparing to pump it out. The site is in good condition and all fluids are contained. No violations.
1940389138 2015-11-12 Production Wells No Violations I was on location to follow-up on a tank overflow that occurred on 11/8/15. The steamer truck was scheduled for this morning and apparently it had broken down and wont be available until next week. The Enervest water truck was on location and I had him vacuum up some accumulated rain water inside the tank battery. I investigated the perimeter of the containment dike and found no indication that the salt water was leaking or leaching from the tank battery. I will continue to monitor this location.
1959630652 2014-05-21 Production Wells No Violations I received a call from Don Nething (tenant farmer) stating that access to the field east of this drilling location was blocked by stockpiled topsoil and a large drop off of the drilling location. He said that he needs to get in there in the next week. I told him I would speak with Enervest about the issue and have them fix it. I inspected the location and found that there was no access to the field. I spoke with Kyle Ahbel (Enervest) and explained to him the situation. He said he would get someone over there right away to fix the access issues. Kyle called me later in the day and informed me that the area has been regraded and access to the field has been restored. No violations.
