Enervest is re-covering the soil pit and will be re-evaluated at the beginning of July. The 8.625 annulus continues to bubble with the 50 lb pop-off valve installed. I spoke with Kyle Ahbel and suggested they try a 25 lb pop-off or try to tighten the head on the 8.625 casing. I also requested that the dike around the well head be excavated and filled with gravel and clay soil to prevent water from ponding and saturating the inside of the dike.
0472934054 2012-07-13 Production Wells No Violations Follow-up inspection of the remediated soil that was spread across the location on 6/26/12. The remediated soil has been spread and the location has been leveled and seeded. The soil is in good condition and there are no visible signs of oil leaching from the remediated soil. The dike around the well head has been repaired and filled with gravel. The annulus is no longer bubbling and the pop-off valve is functioning properly. All issues from the 2/22/12 oil spill have been addressed and the impacted area has been successfully reclaimed. No violations.

0996278129 2012-06-26 Production Wells No Violations I met with Enervest on location to examine the remaining, affected soil pile. The soil was in good condition (no free oil) and remediated. I advised them to mix in more lime and peat and gave them permission to spread the remaining remediated soil on location and reclaim the diked holding area. The bubbling around the well head continues and I will address this issue with Kyle Ahbel (Enervest).
1083045706 2012-05-21 Production Wells No Violations I was on site today to follow up on the reclamation of an oil spill that happened earlier this spring. Operator has contaminated soil stockpiled on site. Soil has a liner under it and is sealed with a liner covering the stockpile. I verbally reminded Woody McDaniel from Enervest that restoration needs to be completed and contaminated soil needs removed. An e-mail will be sent explaining the work that needs to be completed on location. Pictures will also be sent with the e-mail. Work needing completed: Contaminated soil removed and disposed of at an approved landfill. Affected area from the spill needs soil amendments added, tilde, seeded and fertilized to establish vegetation and stabilize the site. The annulus on the well is open and is bringing up fluid. Annulus needs plumbed above grade and a 50 psi pop-off installed on it. The pop-off will hold pressure and prevent fluid from coming to surface and release annulus gas in a controlled manner. Lease road will need to be repair graded.
1331038049 2012-02-22 Production Wells No Violations I was on site at the request of the operator due to an oil spill at this location. The spill did not affect perennial streams in area. There was a fine mist of oil on an ephemeral stream to the north of the wellhead. The operator constructed a siphon dam at the head of the intermittent stream that the ephemeral stream discharges into as a precautionary measure. Approximately 5 barrels of crude oil and brine were released from a 1/2 line coming off of the well head and going to a pressure regulator. An aproximately 8,000 square foot area was affected by the release. The operator constructed a berm around the spill area. Grossly contaminated soil will be temporarily stockpiled on a liner and covered until weather conditions allow for the removed from location. Lightly contaminated soils will be bio-remediated on location. Soil will be treated with Peat-Sorb, Gator and lime. New soil will be placed in the excavated ares and then seeded and mulched. The siphon dam will remain until reclamation is completed.

1635830709 2012-06-04 Production Wells No Violations Follow up to 02/22/2012 oil spill at this location. I met with Kyle Ahbel, Enervest Field Supervisor (330-365-7954), on location to check the soil remediation progress and address the leak around the well head. A 1 deep hole was excavated around the 8.625 casing to expose the annulus valve. The valve was open and the end of the valve was plugged with clay and rock. The blockage was removed and the annular pressure dissipated quickly. The bubbling stopped immediately and a 50 lb pop-off valve was installed to remedy the issue. A longer riser pipe needs to be installed to keep the valve above ground level, prior to filling in the hole. The remediating soil pile was uncovered and the top half of the pile was in good condition (no free oil) and removed to be spread. The lower half of the pile requires further mixing and bio-remediation or removal/disposal in a landfill. Kyle elected to bio-remediate for another month and was given permission to leave the soil uncovered until 06/08/2012 to allow it to air out and then be covered again. The siphon dam was in good condition and funtioning properly. No violations.
1772995934 2013-10-09 Preliminary Restoration No Violations I was on location to inspect the preliminary restoration status of this site. This location was completely reclaimed on 7/13/2012 following an oil release. The entire location has been seeded and graded and there is no equipment on site except for that used in the production of the well. The seed bed is well established and there are no signs of active erosion. The well is operating with the aid of a pumping unit. It appears that the unit is no longer plumb and the polished rod is vibrating the stuffing on the upstroke to the point that the box is begining to leak. The dike in front of the unit contains water with an oil sheen on top and is visibly bubbling between the production-surface casing annulus. I spoke with Kyle Ahbel (Enervest) about the issues and he is going to get them fixed. I suggested putting a lower rated pop off valve on the annulus to get rid of the bubbling. He asked if he could plumb it to a drip bottle and then to the motor on the pumping unit and I told him that would be fine. No violations. Preliminary restoration is approved.

2006347186 2012-05-22 Production Wells No Violations Follow-up to oil spill investigated by Tom Hill on 02/22/2012. I found natural gas bubbling around the well head and the valve from the 8.625 casing is below ground. Oil-stained peatsorb is present in the dike surrounding the well head and this dike has been cut. The stockpile of impacted soil, which is being bio-remediated on-site, is covered, lined, and diked. The siphon dam is in good working condition. The lease road through the woods is in poor condition. My findings were reported to Tom Hill and he will speak with Woodie McDaniel (Enervest).
