0283201515 2015-09-10 UrbanProduction Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well is equipped with an electric powered pumpjack. The tank battery consists of two (2), 100 barrel production tanks, a 50 barrel brine tank, and a mechanical separator. There is a dike around the tank battery. Locked, chain link fences with slats surround the well and tank battery. Identification signs are posted at the tank battery, well, and lease road entrance. There are plugs on the load lines. Urban, safety equipment is installed. The lease road is in stable condition with a locked gate at the entrance.
0419191738 2010-07-15 Production Wells No Violations I received an email from Dr. Meyer with an erosion complaint that he thinks originates on the drill site and is impacting his wetland. This inspection found that the area affected by the drilling is well vegetated with no evidence of erosion. The only bare ground on the site are the lease road tracks leading to the wellhead and they show no sign of eroding. This response was sent to Dr. Meyer with the suggestion that if the sedimentation continues, he contact Portage Soil & Water.

1127380972 2006-01-25 Production Wells No Violations I received a call from the adjoining property owner south of the lease road. The meter run has been set on the south side of the lease road adjacent to the neighbors house and she was concerned. I explained that the meter run location is usually dictated by Dominion and that there is little chance of fire or explosion. However, the meter run is approximtely 30 feet from her house and I will talk to Gonzoil about using better judgement when setting the run in the future.

1218081505 2006-10-23 Production Wells No Violations On October 19, I received an email from Christopher Meyer, at 140 Hanes Road in Aurora, requesting a site inspection of this well to determine if this drill site is source of sedimentation affecting his property. This inspection found that the drill site has been graded, seeded and growing. There are ruts on the lease road west of the tank battery caused by a contractor hired by Gonzoil to install electric service to the pump jack. There was no evidence of erosion gullies or wash-outs originating on the drill site. A majority of the silt fencing installed for drilling is still in place. Gonzoil will repair the ruts and monitor the site for erosion. The results of this inspection were sent to Dr. Meyer on October 23.

1728680611 2018-04-26 UrbanProduction Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the lease road gated and locked. The wellhead is fenced and screened, and the man door is locked. The wellhead is equipped with 2.375 tubing and rods and operating with the aid of an electric powered pumping unit. A gauge on the tubing indicates a pressure of 60psi. This well produces to a diked tank battery to the east along the lease road. The battery is fenced and screened, and the man door is locked. The battery contains one (1) 140 barrel tank, one (1) 100 barrel tank, a 35 barrel poly tank, and a horizontal separator and meter run. The tank hatch lid is closed, the stack is equipped with a relief valve, and fumes are vented through a 42 gallon charcoal filter. The required ID is posted at the wellhead, tank battery, and at the lease road entrance. No violations.
