0115356680 2013-07-26 Production Wells No Violations My final restoration inspection showed that this permit number is a producing well. The well ID card photographed (#4 RR) suggests that this well was deep-plugged and is now a Rose Run well. A review of the records revealed that the well was PLUGGED AS DRY IN JUNE, 2002. REOPENED FOR PRODUCTION IN JUNE, 2003. Also photographed are an old separator, line and additional material left on site. Final restoration inspection not necessary and the status needs to be amended to PR.
0160576163 2013-09-25 Preliminary Restoration No Violations This well was listed as plugged and abandoned in RBDMS and required Final Restoration approval. I reviewed the well files and found that this well was plugged as a dry hole in June of 2002 and reopened in June 2003 and has been reporting production since. My inspection found this Rose Run well operating with the aid of a pumping unit. The tank battery was diked and contained 3-210 barrel tanks and a horizontal separator. There was an old separator sitting on location. I e-mailed Steve Opritza about the discrepency and he has changed the status to producing in RBDMS. This well now requires preliminary restoration approval. Preliminary restoration is approved. I will talk to PEP about the old separator on location. No violations.
0465319308 2015-10-26 UrbanProduction Wells No Violations Follow-up to an inspection conducted on 9-2-15. The concrete pad of the pumping unit that was coated in motor oil has been steam cleaned. The affected soil behind the unit has been removed and a containment dike has been constructed around the pumping unit. The issues I found previously have been resolved. No violations.

1944042368 2015-09-02 Production Wells No Violations I inspected this Rose Run well while following up on Complaint #5108 concerning the Abbe #3 (PN#24247). The landowner, Mr. Plymale, had mentioned that oil was also leaking from the pumping unit motor on this well when I spoke to him earlier in the day. My inspection found the well equipped and producing with the aid of a natural gas-powered pumping unit with 130psi on the 2.875 tubing and 100psi on the 4.5 production casing. The unit is spotted on a concrete pad and the rear of the pad (8 X 5) from below the motor to the north corner of the concrete is stained with motor oil. The oil has migrated and stained the ground off the north corner of the pad for 10. The pumper, Steven Pero, arrived on location to change the meter chart and I spoke with him about the issues I found at the pumping unit. He told me that he would get a contractor in to steam clean the pad and remove the affected soil. I instructed him to either use pads to catch the motor oil where it is leaking and replace them as necessary or use a bucket to collect the leaking oil and empty it as needed. I will follow-up in thirty days and issue a compliance notice if the remedial and recommended actions are not taken and implemented.
