001216i145 2002-01-11 Preliminary Restoration Violations Noted Met on site with Mr. Dave Caywood. Mr. Caywood is concerned that the company has not performed work that they were supposed to when the well was drilled. Met on site also with pumpers Rod and Mark Smith. Mr. Caywood was informed that the Division could not address the problem of the stumps and trees. The violation is failure to fill in a pit, removal of old tires and pollution and contamination around the wellhead. Rod is to remove the oil this weekend. Mark asked for sixty days to correct the pit and tire problem since they may do something with the stumps and trees.

0848842778 2017-08-24 Production Wells No Violations I conducted a status check inspection of this Rose Run well. My inspection found the well equipped and operating with the aid of a natural gas powered pumping unit. The well produces to a diked tank battery containing three (3) 210 barrel tanks, a vertical separator, and the associated meter run. A gauge on the separator indicates a production pressure of 110psi. There are two (2) domestic taps past the separator and are in service. ID is posted at the wellhead and tank battery. I found no violations at this time.

1630963677 2004-03-11 Production Wells Violations Noted Status check inspection - the containment dike has been cut with a backhoe, presumably to drain water from the dike. A quantab test of the water indicated that it was fresh water. I called Mark Smith and left a voicemail message that the dike must be repaired as soon as possible.
