The separator/gas production unit has been removed from the fenced area 90 north of the tank battery and there is no associated production equipment on location. Enervest acquired the well in 2007 from Belden & Blake and there has been no commercial production reported for this well since 2002. I will speak with Enervest about the status of this well. |

Enervest Operating acquired the well in 2007 and has reported zero (0) barrels of oil and zero (0) Mcf of natural gas production from 2007 through 2016. The wellhead consists of 4.5 production casing equipped with a 4 plunger (Concoyl) lift system. The wellhead was fenced with the ID posted and the valves on the flanged casing head and flow line were open. There is a partially fenced area to the north east that is overgrown and contains an old domestic meter, regulator, and a one (1) inch supply line. The tank battery was found badly overgrown with weeds and small trees and the fencing is missing on the north and west sides. The tank battery was diked and contained one (1) 210 barrel tank and one (1) 70 barrel tank. I found no gas production unit or separator on this location. I spoke with Joe Teppert (Enervest) to inquire about the status of this well. Joe told me that he would look into it and get back to me. I will wait for Enervest to respond. |

There is a two (2) inch flow line riser at the location of the separator and several pieces of one (1) inch plastic line exposed between the former separator location and the tank battery. The 210 barrel tank and 50 barrel tank remain on location, have been moved, and the dike walls have been knocked down. Trees and brush have been pushed into the tree line and there is a pile of debris containing stone, dirt, and cement from the former post holes to the south of the tanks. Final Restoration is due by mid-August. This Compliance Notice will be closed when Final Restoration is achieved. |

Enervest Operating will be required to submit an executed landowner waiver (Form 5) to leave the flow and sales lines buried, and for the lease road to remain. No violations. |

It is determined that the well is currently incapable of production. The well owner/operator shall submit a written plugging schedule to Michael Dinsmore at 3575 Forest Lake Drive, Suite 150, Uniontown, Ohio 44685-8116 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |

They tagged the bottom of the hole at 7,200 and the perforations are open (7,099-7,044). The intervals to plug were reviewed and agreed upon and Chipco rigged up and began running 4.7lb/ft., 2.375 tubing into the hole in preparations to set the bottom plug tomorrow (5/16/18). No violations. |
