0608699617 2004-02-12 Production Wells Violations Noted Spill remediation status check - sorbent pads and peatsorb area still around the wellhead. Waiting for the snow to melt to assess the impact boundaries.
0716579213 2018-06-06 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. The wellhead is fenced and diked and a Murphy Switch gauge indicates a pressure of 50psi on the tubing. There are two (2) domestic meters with regulators plumbed to the 4.5 production casing port. This well produces to a diked tank battery at the east end of the lease road and is shared with the Jones #1 (PN#23739). This well produces through a dedicated horizontal separator and into two (2) dedicated 100 barrel tanks. The battery is fenced and the required ID is posted at the wellhead and at the tank battery. No violations.

0846210142 2004-02-05 Production Wells Violations Noted Status check inspection. Stuffing box failure resulted in a one barrel spill around the wellhead. Wade Pol Contracting responded to and started to remediate the spill. According to Mitch Fitch with CGAS, remediation will continue when the weather permits.
1308758293 2011-06-20 Production Wells No Violations I inspected the pumping unit area and found the oily top soil removed and new top soil spread out, seeded and strawed. Inside the fence around the front of the pumping unit is cleaned up and new rock laid down. This NOV is resolved.
1673109206 2011-06-16 Production Wells Violations Noted Bob Worstall called me about an oil spill at this location. I arrive some 30 minutes later. I met with Envervest. The clean up had started. The pumping unit stuffing box had blown out and spread 3 barrels of oil in front of the unit and in the grass to the north of the unit. The top soil to the north will be removed today and new top soil brought in. The area will be seeded and strawed today. I will check on this well tomorrow. This NOV continues.
