0871780036 2012-05-21 Production Wells No Violations On 5/21/2013, I inspected the Northwood Energy sales line along German Church Rd. by Walborn Reservoir. There was no oil or sheen visible on the water inside the siphon dam. I spoke with Chris Bowels ( Northwood Energy ) and he stated that they were going to insert a 2 plastic line inside the 3 steel pipeline. They are planning on starting this week and he will call when they start.
0888304939 2012-05-10 Production Wells No Violations On 5/9/2012, Dave Clause Called John Fleming saying that he had received a call from Greg Gawan that a leak had not been repaired on German Church Rd. I left my house at 5:00 pm I called Greg Dawan to get the location of the leak. I called Chris Bowels a representative of Northwood Energy he meet me at the location, he informed me that this is a 3 bare steel pipeline transportation natural gas, there is a small stream and road ditch that runs into Walborn Reservoir there was a small leak in the ditch that has water running through it, we walked the ditch to the lake found oil along the ditch line no oil had got to the lake it was contained by vegetation . I instructed Chris to put out some oil absorbent booms and absorbent pads in the standing water. At that time Chris Scofield a representative for Northwood arrived on location I instructed them about getting the oil along the effected area cleaned and install a siphon dam in the ditch, Chris Scolfield called Don Cappaoletti he came to the location we talked about what needed to be done. On 5/10/2012 they reinstalled and replaced absorbent pads and booms and laid some peat absorbent on the vegetation a small of oil and an small shine of oil on the water in the ditch there was no oil show on the lake. They Installed a siphon dam with 2- 6x 10 pvc pipes the size of the dam is 8 wide 20 long 24 to 30 deep. There is 2 booms installed down stream of the outlet of the dam and also one 40 boom and also absorbent pads on the beach to collect any oil that my get by. As of 5/11/2012 Northwood has changed pads only and will do so daily until it is clean. The amount of release is estimated at 1 gallon. This pipeline is known as the City of Alliance Pipeline. GPS location of leak 40.98818 / 81.17846

On 5/16/2012, I received a call from Bart Ray of the Ohio EPA had been to the site and was satisfied with what has been done. He told me that he would turn this over to us ODNR Division of Oil and Gas. On my inspection I did not see any oil at the dam or down stream of the dam.
1275449381 2015-10-15 Production Wells No Violations I conducted a status check of this tank battery while conducting annular disposal inspections. The battery is diked and contained one (1) 210 barrel tank and a horizontal separator. There are two (2) brine vaults on the east side of the battery and a badly rusted 210 barrel tank to the north of the vaults. The rusted tank is empty and not in use. It may be a remnant of the production equipment used for the City of Alliance #s 2 & 4 (23565 & 23590) that were plugged and abandoned in 1990. I will talk to Northwood about the old tank. I did not inspect the wellhead at this time. No violations.

I arrived at the location of a leak being repaired by Dave Strack a contractor for Northwood Energy there was no oil show on the ground. The location of the leak is GPS 40.98817 / 81.17458. Approximate 200/225 east of the Walburn Reservoir I walked to lake there was no oil contamination.

1338475597 2012-05-30 Production Wells No Violations On 5/25/2012 I received a call from Dave Strack a contractor doing work for Northwood Energy, has started inserting 2 SDR 11 plastic pipe inside the exciting 3 steel pipeline. On 5/30/2012 I went to the location of the leak GPS 40.98818 / 81.17846, they have passed through this area there was no oil in the siphoning dam. They are on the east side of the bridge. I called Chris Bowels and instructed him to leave the dam in place until we notify him. This is known City of Alliance pipeline.
