0115331628 2015-08-03 Temporary Abandoned Annular Disposal No Violations I inspected this well to check for the use of annular disposal. This wellhead is equipped with a natural gas powered pumping unit and has 50psi on the tubing according to the pressure gauge. This well produces to a diked tank battery shared by PN#23243 and contains four (4) 100 barrel tanks, two (2) horizontal separators and meter runs, and a brine vault. The proper ID signage is posted on each set of tanks. I found no evidence of annular disposal being used at this time. No violations.

1420552919 2014-12-15 Production Wells No Violations I received a call from Dan Horning (contract pumper) on 12/14/14 at 5:33pm that they had discovered a flow line leak in the woods between the wellhead and the tank battery around 5:00pm and have shut the well down to stop the leak. At this point, they did not think any Waters of the State were impacted. I was on location the next morning (12/15/14) and met with Tom Fischietto (President of Bancequity) and Dan Horning. I was informed that oil was found ponded in a drainage ditch/creek that empties into Walborn Reservoir and that no oil had reached the reservoir. I assessed the release and found an area, 20 in diameter above the flow line, saturated with oil. An estimated 5-10 barrels of oil was released during the incident. We walked the creek and there were several pockets where oil had collected behind trees and branches. At the farthest point where debris had blocked the flow of oil (700-800 downstream from the leak), an earthen siphon dam was being installed with 2-15 corrugated pipes. I instructed them to install booms further downstream and to construct another siphon dam as a precaution. I walked an unknown distance along the meandering creek and found no evidence that the release had impacted downstream from the first siphon dam or the reservoir. I instructed Tom to call the OEPA spill reporting hotline as this was a reportable spill. As Tom reported the spill, I called Kurt Kollar (OEPA) to notify him of the incident. He said he would call Wade Balser (OEPA) and get him in route to the location. Scott Miller Contracting arrived on location with equipment to assist in clean-up operations. Wade arrived on location and the OEPA assumed authority over containment and remedial operations. I will continue to assist the OEPA in monitoring the location through the spring before containment barriers can be removed and inspect for additional corrections.
1884794399 2005-04-07 Production Wells Violations Noted Received a call from Portage County Emergency Management reporting an oil spill near this tank battery. I contacted Randy Hall who stated that they had a flowline leak just south of the tank battery which they have been working on. The spill came to surface above the flowline and ran approximately 200 feet down gradient to the west. Additional corrective action is necessary.
