0671091011 2016-01-26 Temporary Abandoned Annular Disposal No Violations I was on location to inspect the well for the use of annular disposal. My inspection found the well equipped and operating with the aid of an electric pumping unit. The wellhead was fenced and the proper ID was posted on the pumping unit. There is a domestic hook-up at the wellhead tied into the 4.5 production casing with two (2) regulators. A gauge indicates 11psi on the domestic tap. The well produces to a tank battery shared by the Hillegas #s 2 & 3 (PN#s 23002 & 23003). The tank battery contains one (1) 210 barrel tank, a 35 barrel poly tank, a horizontal separator, and the associated drip bottle and meter run. The ID sign properly indicates the permit #s for all the wells producing to this battery. The containment dike is badly worn on the south, east, and west sides and I will talk to Enervest about building them up for required spill containment. I found no evidence of annular disposal being used at this time.
0931790705 2017-02-23 Production Wells No Violations I received a call from the Emergency Operations & Response Duty Officer, Trish Cartmell, at 10:33am to inform me of one (1) barrel flow line release at that had entered a drainage ditch. I arrived on location at 11:15 and met with DOGRM Emergency Response Coordinator Fred Romeo and Enervest Field Supervisor Kyle Ahbel to assess the release. The leak occurred at the edge of a ditch between the wellhead and tank battery at GPS coordinates 41.12331, -81.06881 (NAD83). The release travelled 600 to the northeast, passed through two (2) culverts, entered the woods, and bottomed out in a grassy marsh area. Booms have been installed in the ditch and Enervest was using their water truck to vacuum oil out of the ditch. There were small patches of oil residue in the marsh area and an examination of a third drainage culvert that drained the area found no evidence that oil had migrated past this point. Scott Miller Contracting arrived on location and Wade Balser (OEPA) arrived and took over as On Scene Coordinator. I inspected the wellhead and found it fenced and equipped with an electric powered pumping unit. The well is currently shut-in and the required ID is posted on the fencing. A domestic tap plumbed to the 4.5 production casing is still supplying house gas. The OEPA is overseeing the clean-up and remediation of the spill. I will check back later on progress.

I contacted Tim McConaha with Enervest and requested that the leak be corrected. He in turn contacted the pumper to replace the valve.
