The liner was then covered by a second layer of sand approximately three inches thick. Several inches of bank run pea gravel was then placed over the containment floor. Also on this day, the pump house was finished and secondary containment around the pumphouse and the load lines was finalized. A sump pump and crock will be placed inside of the pump house with any collected fluids directed to the loading pad collection system. Any load line will likewise be directed to the loading pad system.

As noted previously, two of the tanks have been overfilled. The tanks must be emptied, removed and pressure tested. Impacted material under the tanks must be removed and properly disposed. The tank battery must be properly lined prior to reconstruction.

Withing the next two weeks, a MIT will be performed on the disposal well and a pressure test will be completed for the injection line from the pump house to the well. The injection line was replaced during this reconconstruction.

0694260874 2008-04-08 Salt Water Injection Well No Violations BDB Energy ran a new Weatherford 43AD.0012 packer with tested 2-3/8 tubing. BDB mixed five gallons of inhibitor in a 70 bbl water truck and circulated the mixture down the annulus and up the tubing with a low rate treating truck. Inhibitor mixture circulated to surface. The packer was set in tension at 3675. After the packer was set, the annulus was pressured up to 1020 psi. After 5 minutes, 1020 psi, after 10 minutes, 1020 psi and after 15 minutes, 1020 psi. The MIT passed.

0845726394 2005-05-31 Salt Water Injection Well No Violations This disposal well has been sold to Annarock by D&L Energy. Annarock is in the process of satifying the outstanding Chiefs Order. The tank battery has been dismantled, a 30 mil liner has been installed and the pumphouse has been rebuilt. On this date, Annarock performed a mini-test on the annulus and a MIT on the new injection line from the pumphouse to the well. Both tests passed. The mini-test pressured the annular space to 280 psi with no loss in 15 minutes. The injection line MIT pressured up to 910 psi with a loss of only 10 psi in 15 minutes. The loss occurred in the first five minutes and was probably associated with air trapped in the system. The system should be ready for turn on within the next week. At that time, the CO will be satisfied.

One of the 210 bbl tanks has been overfilled, causing a spill inside of the dike. The dike floor and sides are constructed with native material which consists of sand and gravel. There is no documentation in the file that indicates that the dike has been lined. D&L is ordered to 1) replace the valves so that pressure reading can be obtained 2) provide documentation proving that the containment dike is lined and 3) if such documentation can not be provided, the storage system must be dismantled, contaminated soil removed and disposed of and a proper liner installed and witnessed by MRM personnel.

On Friday May 13, Annarock began satisfying the outstanding CO by dismantling the tank battery and removing impacted material from under the stock tanks. Supr. Cheslock was onsite to witness this activity. On Monday May 16, Annarock was back on site to begin construction of the required containment dike. The dike dimensions are 40x60x4. The dike is being constructed with soil from both onsite and offsite sources. The dike was completed and the floor was being prepared for the required 30 mil plastic liner.

No leaks. The rig started to hook up for injection. There were no violations at this time.
1224095803 2008-10-15 Salt Water Injection Well No Violations I was called by the DBD rig operator. He said that the tubing had the tubing pressure maxed out at 900 psi. They were going to pull out of hole with the packer and tubing. Then the cased hole would be cleaned out. I checked on the rig. They were starting to pull out of hole with the tubing and packer. There were no violations at this time.
1224899307 2008-10-16 Salt Water Injection Well No Violations The well passed a MIT completed on 10/6. However, when the well was placed back into use, the injection pressures quickly reached the maximum allowed. Annarock decided to remove the packer and try to clean out the hole. At this time, the packer is out and the tubing string is ready to run in to circulate the hole.

While on location, there were two (2) brine hauler trucks from Rev Hoopes Trucking with no UIC numbers displayed. One truck was completely empty and the other had just finished. I spoke with both company truck drivers concerning the missing UIC numbers. Both company truck drivers stated they were unaware that the brine numbers were required to be displayed on the trucks. An UIC number of 25-439 was on their paperwork for Diamond Disposal. According to the facility operator, Rev Hoopes Trucking delivered a total of 125.55 bbls. of brine.


I told the facility operator and spoke with Matt Kleese, owner of Diamond Disposal, to turn away all brine hauling trucks that do not have the proper UIC number displayed.

1533824995 2018-08-09 Salt Water Injection Well No Violations I was on location as a request from ODNR Emergency Operations Coordinator Southeast Region Jason Blocker due to receiving a call concerning a brine spill within the diked tank battery from a former employee of Diamond Disposal. I found upon arrival a facility operator on location, a Eureka brine truck off-loading, and a maintenance crew making repairs to the injection pump. My initial inspection of the wellhead found it not operating. I gauged the injection tubing and annulus while the wellhead was not operating and obtained reading of 380 psi on the injection tubing and 190 psi on the annulus. I observed no spills or leaks within the diked tank battery. There was no visible evidence on the side of the storage tanks or surface area that Diamond Disposal had exceeded the capacity of the storage tanks. I took a brine sample from the Eureka brine truck (UIC #25-

1854229034 2008-11-17 Salt Water Injection Well No Violations BDB Energy was on site to run the Weatherford 43AD.0012 packer back in the well. Prior to resetting the packer, the well was treated with 1,000 gallons of acid. BDB ran 117 joints (3677) of new 2-3/8 upset tubing. The packer was set in tension at 3679. One 31.2 tail pipe was placed below the packer. Five gallons of inhibitor was mixed in 85 bbl of fresh water and pumped down through the tubing and up the backside. The inhibitor mixture circulated to surface before the packer was set. The treating truck then pressured up the annulus between the tubing and the production casing to 1,010 psi. There was no pressure loss after five, ten or fifteen minute intervals. The MIT was successful.
