0001216i26 2001-09-12 Production Wells No Violations Well is now operating.
0836239147 2011-11-21 Production Wells No Violations I received a email from Columbus office about a complaint. The complaint number is 3453. I inspected the well and the tank battery. The well had not been producing. RBDMS showed no production since 1998. I called Mark Spare (330-466-2414) about the well. Mark said he and James Smail would get back to me in a few days after they discussed what they wanted to do with the well. I tolk Mark to either produce of plug this well. I will call back 11/28/11 for the status. I called Jim Hammer, a Freedom Township Trustee, to let him know what the status was. I will call him next Monday with an update. This complaint continues.
0839343370 2012-10-09 Final Restoration No Violations I received a call from Charlie VanSteenvert (8/27/12) with the Freedom Township Road Department concerning a 300 barrel frac tank sitting near the entrance of the lease road. He uses this area to dump dirt for the Township and wondered if I could call somebody to get it moved. This well was plugged and abandoned on 5/17/12 and final restoration is due by November 17, 2012. I called Jim Smail and left a message about getting the tank moved. Jim sent me a text several hours later and told me he had a call in to R.C. Pander to move the tank and would let me know when it would be moved. Jim texted me the next day (8/28/12) that Pander would be moving the tank today. During my inspection on 10/9/12, I found that the tank had been removed. The lease road was restored and all the surface equipment had been removed except for the ladder from the tank battery, which was laying in the weeds. I contacted Jeff Schonauer (JR Smail) and let him know that the ladder would need to be removed and that a landowner waiver would need to be submitted to leave the flow-line buried in the ground before I would approve final restoration at this location. No violations.
0847665687 2012-05-17 Plug / Plug Back No Violations I was on site today to witness plugging activities. The 4.5 inch casing was shot at 2769 feet. The operator pulled up 20 feet to set the lime plug. (See plug report for plug depths.) During cementing gas kick was circulated out of the hole. The hole had a salt ring. The hole unloaded water and oil, during the unloading the surface casing failed. Gas and fluid was coming up the back side of the surface. When displacement was complete, the casing went on vacuum for a minute. Hole became static, 4.5 inch casing was tripped out to the bottom of the first surface plug. Operator loaded hole. Hole was static. Operator broke circulation, mixed cement, ran cement and then displaced. No lose of circulation when cementing, I left site due to a gas leak in Lake County. Rick Skidmore was on location to witness the last plug.

1337098273 2012-05-15 Plug / Plug Back No Violations On 5/15/2012, I inspected a plug job with DNR Inspectors Tom Hill and Mike Dinsmore. I witnessed the stemming of 2 3/8 tubing, tripping the tubing into the well, setting the bottom plug and tripping out the tubing. Refer to DNR Inspector Mike Dinsmores plugging report.
1337109791 2012-05-15 Plug / Plug Back No Violations I was on site today to check the status of the plugging of the Pochedly #1. DNR Inspectors Mike Dinsmore, Bob Roush and Rick Skidmore were also on location. I reviewed plug locations with the new inspectors. Operator tripped in 2 inch tubing to set the first plug, 50 feet below the bottom perforations. First plug was not going to be bullheaded in. The well had a tight spot at 3500 and 4100. Petroset Cementing pumped on the well to break circulation. Well pressured up to 500 psi and was taking fluid. Petroset obtained circulation, mixed, ran and displaced cement. Operator then began to trip tubing out of hole. Operator to rip 4.5 inch casing on Wednesday and complete plugging. See plug report for specific casing sizes, plug depths, and rip depth.
1337260729 2012-05-16 Plug / Plug Back No Violations On 5/16/2012, I inspected a plug job with DNR Inspectors Tom Hill and Mike Dinsmore. I witnessed Haggen Well Service crew trip the 2 tubing out of the well and weld the well head onto the 8 5/8 casing. Refer to DNR Inspector Mike Dinsmores plugging report.
1337365775 2012-05-17 Plug / Plug Back No Violations On 5/17/2012, I inspected a plug job with DNR Inspectors Tom Hill and Mike Dinsmore. I witnessed Applalachian Well Surveys bond log the well and shoot off the 4 1/2 casing at 2769. Universal Well Services set three plugs: a the Big Lime (2449 to 2749), Surface casing (504 to 304), and Surface (0 to 200). There were 62 full lengths (44.35) and three 10 lengths of 4 1/2 casing removed from the well. Refer to DNR Mike Dinsmores plugging report.
1338317239 2012-05-16 Plug / Plug Back No Violations My inspection today found the operator tripping tubing out of hole. The operator will be completing plugging of the well on 05/17/2012.

1719475391 2012-04-17 Production Wells Violations Noted A follow up inspection of this well found that there has been no activity toward plugging and abandoning this well. The last reported production was in 1996. Discussions with JR Smail personnel in November 2011 indicated that the well would be plugged. Other than applying for and receiving the plugging permit, no action has occurred. Failure to comply with this notice in the time allotted will result in additional corrective action.
2046350503 2012-11-06 Final Restoration No Violations I was on location to check the Final Restoration status of this plugged and abandoned well. The lease road and location have been restored, graded, and seeded. The surface equipment has been removed and the impacted areas have been reclaimed and restored. The first 200 of lease road is still being utilized by the Freedom Township Road Department to store dirt and topsoil. I have received the landowner waiver from J.R. Smail and the Freedom Township Trustees to leave the flowlines buried in the ground. Final Restoration is approved.
