0637627612 2017-02-24 Production Wells No Violations I was on location with Scott Miller Contracting to follow-up on a flow line leak that occurred on 2/17/17. A three (3) foot section of line was repaired on 2/23/17 and I was informed that crude oil saturated dirt and soil was excavated when exposing the flow line and was stockpiled and covered with a liner. Twenty feet of line was exposed today and any impacted soil was stockpiled for removal. An old repair was inspected and the line was taped in several places where the coating was nicked. The remainder of the affected material (two (2) tandem loads) was hauled to the Kimble landfill for disposal and the flow line was buried. Absorbent pads were placed, where necessary, through the area scraped during the initial response and in the siphon dam basin. I will continue to monitor this location.
0840113521 2017-02-22 Production Wells No Violations I was on location to follow-up on a flow line leak that occurred on 02/17/2017. My inspection today found that several loads of limestone have been delivered and spread on the lease road for stabilization so that the tandem dump trucks could safely access the location. I walked the location and inspected the excavated areas to the east and down to the siphon dam. The siphon dam basin has collected runoff from the excavated area and is functioning properly. There is a minor amount of residual oil in the basin and in several low areas through the excavated area that can be removed with absorbent pads. I found no free oil east of the siphon dam. All the affected material stockpiled south of the wellhead has been removed, the liner has been taken up, and the earthen dike has been graded out. I spoke with Scott Miller and he told me that 2.5 tandem loads of material were removed for disposal at Kimbles landfill this morning. There is currently no set time frame for repairing the flow line and will depend on weather conditions. I will continue to monitor the location.
1487792432 2017-02-17 Production Wells No Violations I received a call from Emergency Operations at 10:28am concerning a flow line leak at this location. I arrived on location at 11:55am and met with Kyle Ahbel (Enervest), Fred Romeo (DOGRM-ERC), and contractor Scott Miller (SMC). Four (4) barrels of crude oil were reported to be released in the incident and we walked the location to determine the extent of impact. The affected area consisted of scrub brush and small trees (<3 in diameter) with a ground cover of leaves. The oil migrated down-gradient to the east in an irregular path measuring 200 in length and 20-30 wide along the extent. The oil did not impact the drainage creek to the east or south and did not enter the designated 100 year flood plain. The impact was surficial and SMC began scraping up all the visibly affected material for disposal. The impacted soil, leaves, and brush were stockpiled on a heavy duty pit liner 40 south of the wellhead. A siphon dam was constructed at the eastern extent of the impacted area with a U-shaped berm and 6 PVC pipe. The leak was excavated and exposed 50 southeast of the wellhead at GPS coordinates: N41.24112, W081.11443 (NAD83). The pipe was four (4) feet below grade and was yellow, jacket-coated steel. A 6 long split was found on the underside of the flow line and was approximately 1/8 wide. A clamp was placed on the line to prevent and residual oil from leaching out of the split pipe and the hole was filled back in for the weekend. Enervest is going to replace the bad section of flow line and pressure test the line prior to placing the well back in production. The contaminated material was covered and diked and will disposed of at Kimbles landfill next week. I will continue to monitor progress at this location.
2022041573 2014-04-25 Production Wells No Violations Status check inspection found the well equipped with an electric pumping unit. The unit is fenced and diked, and the gate is locked. ID is posted on the electric box. Gauges indicate 100psi on the casing and 35psi on the tubing. This well produces to the tank battery at PN#22359. No violations.
