1354743863 2018-12-20 UrbanProduction Wells No Violations Follow-up to close out Compliance Notice #1428499574 and Complaint #4991 . My inspection found the wellhead fenced and locked and equipped with a four (4) inch casing plunger. A gauge on top of the plunger lubricator indicates a pressure of 70psi. The 4.5 production casing and lubricator valves are currently closed and the well is shut-in. The required ID is posted on the fencing. Remediation was completed by Wade Pol Contracting in April and May of 2015. The flow line was repaired during the summer of 2015 (per Equity Pumper) without notifying the Division as required by the Compliance Notice. The violations outlined in Compliance Notice #1428499574 have been satisfied and this Compliance Notice and Complaint #4991 can be closed. No violations. Note: The Emergency Well Locator plots this well 900 NNE of the actual location of the wellhead. I will notify Columbus of this discrepancy
1377628320 2013-08-23 Production Wells No Violations Portage County Inspector Mike Dinsmore ask if I would perform a status check on this location. I arrived on location and walked the length of the ditch to the lake, there was no visual sign of oil on the ground but theres a small sheen in the water before the booms, that were installed before it entered in the lake area. I also talked with one of the Golf Pros on location that was marking the area where the clean up had accrued . He stated that they were very pleased with the work that W.Pol Contracting had performed. All wells are shut in for a period of time to make sure after a rain that no oil was in the drain tile.
1378229161 2013-08-15 UrbanProduction Wells No Violations I received a call from Don Toth (Equity) at 11:09am about a flow line leak at this well. He tolld me 5-7 barrels of oil ran into a ditch and then into an irrigation pond at the golf course. He said the oil was contained to the ditch and pond and did not enter waters of the State of Ohio. I suggested he report the spill to the Ohio EPA in the event that the oil does get out. I arrived on location at 11:30am and met with Equity and Wade Pol to assess the spill, containment, and remedial actions. The well operates with a concoil and is currently shut in. The leak occurred 40 west of the wellhead on the edge of a small drainage ditch. The oil travelled approximately 700 down the ditch to the north and east where it emptied into an irrigation pond. The ditch contained oil along its entire course and where the ditch turned east, it widened out and was overgrown with weeds, cat tails, and grasses. The wind had been blowing out of the southeast and kept the oil contained to the northern quarter of the irrigation pond. They had placed a boom in front of the overflow dam and lined the top of the dam with absorbent pads to prevent the oil from migrating any further. The level of the lake at this time was below the top of the dam and no oil had migrated past this point. The contractor (W. Pol) strung together booms to stretch across the pond and keep the oil contained to the north side of the pond. Kurt Kollar (OEPA) arrived on location, assessed the spill and began overseeing the clean up. Kurt supplied some hard booms and plywood. The hard booms were placed across the drainage channel into the pond to keep any more oil from reaching the pond. Plywood was placed in front of the overflow and driven into the ground across the overflow ditch to form a siphon dam. It was noticed that one of the booms across the pond was twisted and would allow a breach of containment so another string of booms was placed across the pond. 1412355172 2014-04-08 UrbanProduction Wells No Violations I was on location to follow-up on a flow line leak that occurred yesterday (04/07/15) and is detailed in Compliance Notice #1428499574. Wade Pole Contracting and Equity were on location continuing with clean-up operations. They have boomed the irrigation pond three (3) times and have most of the free oil contained behind the booms. Reggie Brown (OEPA) had them place a hard boom across the mouth of the inflow to prevent any residual oil from reaching the irrigation pond, and it is functioning properly. Two (2) additional plywood, underdrain-siphon dams were installed north of the drainage culvert and soft booms were staked in front of the culvert under Loomis Parkway. The drainage ditch between the irrigation pond and the source of the leak was free of oil and a siphon dam was constructed just north of where the leak occurred to gather any leaching oil. This clean-up is progressing.
1428499574 2015-04-07 UrbanProduction Wells Violations Noted I received a phone call from Elaine King (Equity) at 1:47pm concerning a flow line leak at this well. She told me an unknown amount of oil had leaked into a drainage ditch and into an irrigation pond when the well was turned on today. She said she had notified the Ohio EPA and that Wade Pol Contracting was on location containing the release. She said the leak occurred in the same location as the release from 8/15/2013. I arrived on location at 2:40pm and met with the pumper, John Gove, to assess the situation. They were currently excavating the area where the leak originated and we walked the ditch and inspected the irrigation pond. There were soft booms and pads in place along the watercourse and at the pond overflow to try and contain the release. Heavy rains this morning have overwhelmed the overflow and water was flowing out of the pond at two (2) other locations on each side of the overflow and were staked with soft booms and pads. The booms and pads were containing the oil but an obvious sheen was migrating past the containment and off location through a culvert pipe. I instructed the pumper and the contractors to get some plywood and construct siphon underflow dams along the watercourse and at the overflow to stop the migration of the sheen and better contain the oil. I inspected the outflow of the culvert pipe 250 north of the inlet and found a small amount of product in an eddy and instructed several of the contractors to stake in soft booms in front of the large culvert pipe where the water passed under Loomis Parkway. I met with Wade Pol where the line was being excavated. Wade told me the plastic flow line is buried 7-8 below grade. The top 6-8 of topsoil was affected above the leak but the rest of the excavated material was in good condition. The ground in the excavated hole was saturated with water and the hole was making water the deeper it was excavated and was not stable or safe to work in at this time. We decided to line the bottom
1449672692 2015-11-02 UrbanProduction Wells No Violations I was on location to follow up on Complaint #4991 and Compliance Notice #1428499574 concerning a flow line leak at this well. All pollution controls (siphon dams/underdrains) have been removed and the affected areas have been cleaned up and remediated. This Complaint can be closed. I spoke with Jim Gove (pumper) and he informed me that the flow line was fixed by Wade Pol during the summer. I was not notified of the repair of the flow line as required by the original Compliance Notice. The well is currently shut-in and not producing. The man door is currently not locked as required and the ID sign is faded. I found a small leak below the 4 plunger flange where it is threaded to the 4.5 production casing. There is 110psi on the 4.5 casing. I told Jim of my findings and he will address the issues immediately. I will follow-up later to make sure the issues have been addressed before closing the Compliance Notice.
