0028071516 2018-06-12 Drill / Deepen / Reopen No Violations I was on location to check progress of pit solidification. Ron Hammen (Lakeside Oilfield Services) was on location with personnel and they have steamed the bentonite mud out of the 500 barrel frac tank and into a half round. The mud will be placed in the lined pit and solidified with the half-round of kiln dust that was delivered to location. Northeast Oilfield Services arrived on location to haul out the 500 barrel tank. Solidification will begin tomorrow morning (6/13/18). No violations.
0530023378 2018-10-31 Plug / Plug Back No Violations I was on location to witness the plugging back of this re-opened Clinton Sandstone well. RB Drilling spotted 2.375 tubing at 2905 (15 above the 4.5 production casing stub) and Formation Cementing (FC) rigged up and broke circulation. They then mixed and pumped 12.2 barrels of cement (verified at 15.5lbs/gallon) and displaced the slurry to 2705 with 10.9 barrels of water. RB pulled tubing to a depth of 2271 and FC rigged up to the tubing. They established circulation and then mixed and pumped 21 barrels of cement (verified at 15.6lbs/gallon) and displaced the slurry to 1921 with 7.7 barrels of water. RB pulled out of the cement and FC cleared the tubing. They pulled five (5) more joints and shut the well in for the evening. The plan going forward is to have RB pull the remaining tubing, rig down, and move off location. Northwood is contracting a drilling rig to come in and whipstock off the plug at 1921 and drill a new hole to the base of the Lockport Dolomite and attempt to produce from the Newburg Porosity Zone. No violations.
0805531364 2018-02-15 Drill / Deepen / Reopen No Violations I was on location to check on re-opening operations. Nicks Well Plugging (NWP) was on location and currently discussing operations. On, 2/14/18, NWP had run the tapered mill to a depth of 3713, inside the 4.5 casing, and encountered an obstruction. They were able to rotate/circulate to a depth of 3720 and the mill started getting hung and it was pulled from the hole. They are currently inside the top of the 4.5 casing with a 3.625 tri-cone bit at a depth 3040 and have hit an obstruction. NWP attempted to circulate and rotate past the obstruction and the gelled pit water became to viscous to pump (fluid loss down hole). They are going to pump the fluid from the pit into the 500 barrel tank and get another load of salt water for circulating. Operations will continue tomorrow (2/16/18). No violations.

Nicks Well Plugging (NWP) informed me that they would be on location this morning to begin running 4.5 production casing. I arrived on location and met with NWP. They informed me that Northwood has decided to bring in a different string of casing for the job and they will not be running casing today. The pipe trailer was removed and the public road was scraped of mud. No violations.
1053727640 2018-01-30 Drill / Deepen / Reopen No Violations Status check inspection of drill-out and re-opening operations. Nicks Well Plugging was on location and servicing equipment. They picked up a stand of drill pipe and began going down hole past plug #4. The gel spacer in the hole was causing buoyancy issues and the stand was rotated and circulated into the hole without issue. They ran 10 more stands of pipe and I left location. They will continue to trip pipe to the top of plug #3 at a depth of 2287. No violations.

1250720474 2018-05-30 Drill / Deepen / Reopen No Violations Status check inspection of this re-opening operation to check the status of the circulating pit. An employee of Lakeside Oilfield Services was on location and attempting to shrink the size of the pit. There is very little fluid on top of the bentonite mud in the pit. The plan was to get the liner folded over to avoid more water from entering the pit from the forecasted rain and solidify the mud when things dry out. I will continue to monitor the pit until it is closed. No violations.
1273892759 2018-01-10 Drill / Deepen / Reopen No Violations I was on location to check the status of site construction prior to re-opening this plugged and abandoned well in an attempt to produce from the Onondaga Formation through the Lockport Dolomite. My inspection found Nicks Well Plugging (NWP) personnel on location building the lease road and drilling area. Northeast Express Services is currently delivering the stone. A mud pump and swivel, a steel half round, the pump truck, and a flat bed with 2.875, 6.5llb/ft., external upset tubing and collars are currently on location. The 8.625 surface casing has been exposed and a four (4) foot riser has been welded to the old surface casing. There is 11.75 drive pipe exposed (not indicated on the completion report) and the annulus between the two (2) casing is filled with cement and solid. The casings are slightly out of vertical and an attempt to straighten and backfill behind the drive pipe will be made tomorrow. No violations.

They are currently at a depth of 2810 and continuing to drill at a good rate. NWP drilled out of the plug at a depth of 2884 and circulated to the top of the fish (4.5 production casing) at 2920. They are going to circulate a bottoms up, pull one joint of pipe, and pack the well in for the evening. No violations.

Lakeside Oilfield Services has moved the mud from the half-round into the lined pit. The 500 barrel tank has been removed from location. Ron Hammen (Lakeside) decided not to put the dust in the pit today due to high winds blowing towards the homes to the east. The pits will be solidified tomorrow (6/14/18). No violations.
1358112950 2018-02-22 Drill / Deepen / Reopen No Violations Status check inspection of this re-opening operation. Nicks Well Plugging (NWP) was on location and running back into the well bore with a 6 skirt and flat mill bit on 2.375 tubing. NWP encountered something 150 above the fish (casing stub) and were able to circulate and rotate through. They reached the top of the fish and were able to swallow up the casing stub and they began to mill. They milled on the casing for 55 minutes and cutting were caught with a magnet indicating that they were, in fact, milling on the casing. They plan on milling at least one (1) foot and then shutting down for the day. No violations.

1516642589 2018-01-22 Drill / Deepen / Reopen No Violations Status check in inspection of this re-opening operation. Nicks Well Plugging (NWP) was on location and the pit and cellar have been lined. Northeast Oilfield Services delivered two (2) loads of brine to the pit to be used in the drill-out. NWP began drilling the top plug at 12:45pm with a 7.875 tri-cone, button bit and a three (3) inch drill collar. After one (1) hour, the rig had drilled up three (3) feet of cement. The first stand will be drilled and the blow-out-preventer (BOP) will be installed as the drill bit will not fit through the BOP stack. The BOP will be tested tomorrow (1/23/18) prior to drilling out below the top plug. No violations.
1516737735 2018-01-23 Drill / Deepen / Reopen No Violations I received a call on 1/22/18 from Nicks Well Plugging (NWP) to inform me that the pit wall had caved in and that they are currently emptying the pit. I met on location with NWP this morning and inspected the pit. The eastern side of the pit had slumped in on each side of the flow ditch due to ground saturation from the melting snow and ice. Northeast Oilfield Services vacuumed out the remainder of the water in the pit for disposal. It was decided to fold the liner in, recut the dike walls with less angle, and re-line the pit with a new liner. A new liner was delivered to location and installed in the pit. The pit was filled with water and drill-out operations re-commenced at 2:30pm. NWP will drill until dark and shut down.
1516891488 2018-01-24 Drill / Deepen / Reopen No Violations I was on location to observe drill-out operations. Nicks Well Plugging (NWP) drilled the first stand down until there was six (6) feet of drill pipe to go. They set the bit on bottom and began rigging up the blow out preventer (BOP). A six (6) inch drill collar was brought to location and they decided not to use it at this time due to the good drilling rate they had established over the last hour. The BOP was tested for one half hour at 350psi and the test passed. There were no observable leaks during the test. NWP drilled the rest of the stand down (approximately 30 into the top plug), flushed the hole, raised the drill pipe and bit, and shut down for the day. Drill-out operations will continue tomorrow (1/25/18). No violations.
1516911793 2018-01-25 Drill / Deepen / Reopen No Violations I was on location to observe drill-out and re-opening operations of this plugged and abandoned well. Nicks Well Plugging was on location and drilled out of the bottom of plug #5 (50-0) at 40 with two (2) 2.875 drill collars measured from 5 above ground level (GL). They tagged the top of plug #4 (392-290), 10 low at 300, drilled one stand to 330, and shut down for the day. Good circulation was maintained throughout the operation. No violations.
1516978928 2018-01-26 Drill / Deepen / Reopen No Violations I was on location to observe drill-out and re-opening operations of this plugged and abandoned well. Nicks Well Plugging (NWP) was on location and continued to drill out plug #4. I informed NWP that they would come out of the bottom of the surface casing at 342 (GL) and not 352 (KB) that is reported on the completion report. Also, I informed them that there is 11 hole from 342 to 368 (GL) based on the Division inspectors record of cementing and mudding and that the remainder of the plug would be inside a 7.875 hole. NWP drilled one (1) stand to a depth of 364 and we caught and examined cuttings through the next stand to make sure they did not drill off the cement plug. They drilled to a depth of 380 and shut down for the weekend. No violations.
1517258083 2018-01-29 Drill / Deepen / Reopen No Violations I was on location to check the status and progress of re-opening and drill-out of this plugged and abandoned well. Nicks Well Plugging (NWP) continued to drill on plug #4 and I continued to assist in verifying cuttings were, in fact, cement. They drilled out of the bottom of plug #4 at a depth of 412, briefly lost circulation, took a gas kick, and closed the pipe rams. The kick was bled off through the blow line and dissipated quickly. They are currently installing a stripping head and plan to rotate in one (1) stand of drill pipe and shut down for the evening. No violations.
1517854037 2018-02-05 Drill / Deepen / Reopen No Violations Status check inspection of drill-out operations. Nicks Well Plugging (NWP) was on location and they tagged the top of plug #3 (Big Lime) at a depth of 2257 (30 high). NWP drilled through the bottom of the plug at a depth of 2413. They circulated a bottoms up and no gas kick was observed. NWP is going to trip in to the top of plug #2 and shut down. The pit is in good condition and a 500 barrel tank has been spotted to contain any extra fluids. No violations.
1518618203 2018-02-13 Drill / Deepen / Reopen No Violations Status check inspection of re-opening operations. Nicks Well Plugging (NWP) attempted to wash over the 4.5 production casing stub at 2920 with a 6 skirt and flat mill bit on 2/9/18 and were not able to get over the casing. Today, NWP has tripped in the hole with a 3.5 tapered mill bit to try and get inside the casing stub. They rotated and circulated down and were able to get inside the 4.5 pipe without hitting any obstructions. They continued to rotate and circulate the mill down the casing and are going to tag the top of the Clinton Sandstone plug which should be at a depth of 3911. They reached a depth of 3518 and shut down for the day. They will continue to trip in to the bottom plug tomorrow (2/14/18). The pit is in good condition and has plenty of freeboard. No violations.
1520537086 2018-03-08 Drill / Deepen / Reopen No Violations I was on location with Corey McCudden (Northwood), Nicks Well Plugging (NWP), and the Baker Hughes Tool man to witness the setting of the casing patch. NWP ran used, 10.5lb/ft., 4.5 casing to 2920 with three (3) 6.25 bow spring centralizers 40 above the patch and on every other joint. They attempted to get on the casing stub with the patch for three (3) hours without success. They decided to pull the casing and the tool to examine the patch. The overshot lip on the patch was dented and the edges of the patch were marked. The grapple had unseated and the inside of the patch showed no marks indicating that it never got over the casing stub. The Baker tool man was going to re-dress the patch and re-set the grapple to make another attempt tomorrow (3/9/18). No violations.
1521468442 2018-03-09 Drill / Deepen / Reopen No Violations I was on location with Corey McCudden (Northwood), Nicks Well Plugging (NWP), and the Baker Hughes Tool man to witness the setting of the casing patch. The patch was re-dressed and run in the hole. NWP attempted to circulate and rotate the patch onto the casing stub without success and the casing and tool were pulled. Marks on the patch showed that the casing never centered up in the hole to get inside the patch. Northwood is going to shut down and have a meeting to determine a plan going forward. No violations.
1522068250 2018-03-26 Drill / Deepen / Reopen No Violations Status check inspection of this re-opening operation. Nicks Well Plugging rigged down and moved off location on 3/14/18 while Northwood decides on a plan moving forward. My inspection today found the wellhead secured and shut-in. The annular ports on the 8.625 surface casing are equipped with a nipple and ball valve on one side and a nipple and needle valve on the other side. A 4.5 casing stub is in the wellhead and packed off below the ring. The stub is fitted with a 4 x 2 reducer and equipped with a ball valve. The working pit is in good condition and has been secured with safety fencing around the perimeter and has been diked. There is 18-20 of freeboard on the south side of the pit and the solids and gel have settled out leaving one (1) foot of clear water above the solids. The 500 barrel frac tank is not level and there is 24 of space inside the tank on the south (downslope) side. I left a message with Northwood company man Corey McCudden and requested a status update and also requested that the pit level be monitored. No violations.
1523024500 2018-04-06 Drill / Deepen / Reopen No Violations Status check inspection of this currently idle re-opening operation. My inspection found the wellhead secured and shut-in. The working pit is nearly full of water and needs to the water level lowered immediately. I left a message with the company man, Cory McCudden, to get the pit level lowered with a water truck or to pump the water into the 500 barrel tank on location before the weekend. I requested a call back from Northwood to discuss the plans moving forward. I spoke with Corey later in the afternoon and he told me that a water truck was in route to get a load out of the pit. He told me that Northwood has not made a decision on the direction of the project as of today. No violations.
1525101362 2018-04-30 Drill / Deepen / Reopen No Violations Status check inspection of this re-opening operation that was put on hold on 3/14/18 because they were unable to reattach to the 4.5 production casing at 2920. My inspection today found the circulating pit level to the top of the liner. I found no evidence that the water had overflowed the pit wall. I contacted Corey McCudden (Northwood) and requested that the pit level be lowered before any forecasted rains arrive on 5/3/18. Corey told me he had called the water truck this morning to get 150 barrels out of the pit. I will check back on Wednesday to verify the pit level has been lowered. No violations.

Lakeside Oilfield Services has excavated and lined a deeper and wider pit three (3) feet west of the original working pit to allow for the cuttings and bentonite mud to be properly solidified below grade. The mud and cuttings in the original working pit are covered and contained within the liner. There is currently no activity on location and I will check back on progress next week. No violations.

The pit mud was mixed with kiln dust yesterday (6/14/18) and has solidified to the point where the liner can be folded in and the area reclaimed. Lakeside Oilfield Services was on location and are going to complete the pit closure today. No violations.
1529415534 2018-06-14 Drill / Deepen / Reopen No Violations Status check inspection of pit solidification and closure. Lakeside Oilfield Services was on location to begin solidifying the mud in the circulating pit. The kiln dust was transferred from the half-round and into the pit using a track hoe. The dust was mixed in with the mud and was bubbling and beginning to solidify. I requested that the pit be left open to verify good solidification in the morning (6/15/18) prior to closure. There are no violations at this time.

The area has been rough graded and the wellhead is secured by a perimeter of orange safety fencing. The reopening permit is valid until 9/28/19 and I will inquire with Northwood Energy on what their plans are moving forward. There are currently no violations.
1551971672 2019-03-07 Drill / Deepen / Reopen No Violations Status check inspection of this Newburg Dolomite re-opening location. I was informed by Northwood Energy that HAD Drilling is scheduled to move in on this location in the next two (2) weeks to whipstock around the former borehole and complete the well. My inspection today found that stone has been hauled into location and has been spread on top of a liner mid-way to the wellhead. A drainage ditch has been cut along the east side of the location and a culvert has been installed in a swale on the location to control drainage. There were no violations at this time.

1741790619 2018-02-26 Drill / Deepen / Reopen No Violations Status check inspection of this re-opening operation. I arrived on location and Nicks Well Plugging (NWP) was waiting on location for 4.5 production casing to be delivered. The casing arrived and they had to switch out tractor trailers. They brought the casing back to location and I inspected the pipe. I found many heavily pitted and corroded joints. Some of the collars were pitted and there was pipe scale on the inside and outside of the casing. I expressed my concern about using the pipe to NWP and they were concerned as well. They took the casing back to their yard to stack it out in the correct order and I left location. I will ask Northwood to provide any testing data for the casing before it will be approved for use in the well. No violations.

My inspection today found that the fluid level has been lowered to the top of the solids and there is now 18-20 inches of freeboard in the pit. J.Hawk Water Services (UIC#342) arrived on location and removed 75 barrels of salt water from the 500 barrel frac tank. John Myers (driver) said he took 150 barrels off the pit this morning. The water is being disposed of at the H. Myers (SWIW#35), permit # 23542, in Edinburg. Northwood has yet to make a decision on operations moving forward. No violations.
