1445954416 2015-10-02 Final Restoration No Violations I conducted a final restoration inspection for this well. KST Oil & Gas Co. Inc., ceased operations between 1995 and 1996. Jeff Moore (Moore Well Services) used to work for KST and remembers the well being plugged in the late 1980s. Jeff has gone through all of KSTs files and there are no records of the plugging and abandonment of this well. Also, there are no records on file with the Division of Oil & Gas indicating that the well was plugged. I checked aerial photos on the Portage County Auditors website and photos from 1985 clearly show the well location and tank battery. The next year available on the website was 1990, and shows that the well equipment and tank battery had been removed from the location. Research and a field check of the area indicate that this well was plugged and abandoned between 1985 and 1990. I inspected the GPS location of the well and the surrounding area and found no evidence of a producing well or associated surface equipment. Based on my findings and the lack of field evidence that the well exists, final restoration is approved.
