1313150192 2011-08-10 Production Wells No Violations Underground cement tanks are still in place. The dirt pile has not been hauled off yet. The new tank battery is in place. This complaint continues.
1484636929 2011-08-01 Production Wells No Violations The clean up continues. Waldron moved the tank battery south 300 feet. The tank battery is now by the well. Waldron(Ted Cutright) continues cleaning out the ditch area on the north side of the tank battery. Reggie Brown(Ohio EPA) had checked on the ditch earlier in the day. This complaint continues.
1485802697 2017-01-23 Production Wells No Violations I was on location to conduct a status check inspection of this Clinton Sandstone well. My inspection found the well equipped with an electric powered pumping unit. The wellhead was diked and there was 100psi on the 4.5 production casing. This well produces to a diked tank battery shared with the Bossow U #1 (PN#20647) that contains one (1) 100 barrel tank and a horizontal separator. The chart at the separator indicates a differential production pressure of 45psi. The ID posted on the tank needs to be updated to reflect Pin Oak Energy as the current owner of the well and a new emergency number needs to be posted. I called the well pumper, Dennis Kingsbury, and informed him that the ID needs to be updated at the tank battery and that the wellheads also need to be identified. Dennis told me he would update the ID signage this week. I will check back later to inspect for compliance.
1604823629 2011-07-26 Production Wells No Violations I received a call from Ted Cutright of Waldron Assoc.(330-697-7117 about a oil spill from the tank battery. I arrived at the site in 45 minutes to investigate. The spill had spilled into a small ditch. The Ohio EPA ,Reggie Brown, Wade Balser were called out. Reggie walked the small ditch. The spill went down the ditch some 1 mile. The spill happened sometime this past weekend. It rained this past weekend. The clean up started immediately. Ted called for chemical trucks to vaccum the oil from the ditch. The cause of the spill was a two inch ball valve had failed. The threads of the valve pulled apart. I called Rick Simmers to give him a update on this spill. This clean up continues.
1810500273 2011-09-08 Production Wells No Violations The dirt has been hauled away to a certified land fill. The new tank battery has been installed and the clean up is complete. This complaint is resolved.
1952858113 2011-08-23 Production Wells No Violations The cement tanks in the old tank battery were hauled off. The dirt pile is still on the location.
