0033217903 2005-03-09 Production Wells Violations Noted Spill remediation is continuing by replacing sorbent pads and booms as necessary.
0511943897 2005-05-02 Production Wells Violations Noted D&M has been back on site to removed the underground brine vault and additional impacted soil. Enough material has been removed that spill remediation should continue with sorbent pads.
0667713846 2005-02-23 Production Wells Violations Noted D&M was on site to pig the flowline and reset the stock tank near the well. Remediation will continue as weather permits.
0757158384 2006-01-10 Production Wells Violations Noted The violation has been corrected. Any remaining residual oil will continue to degrade naturally with minimal environmental impact.
0953916793 2005-04-01 Production Wells Violations Noted Mr. Wise or his contractors need to change the sorbent pads more frequently. Now that the ice has melted, free oil is starting to move.
1109259421 2005-02-15 Production Wells Violations Noted Received a call from Larry Wise on Saturday, February 12 reporting a flowline leak on the Clinton well he owns on his property. The leak impacted the soil and a nearby wetland. D&M Contracting was on site to remediate the spill.
1110804825 2005-03-07 Production Wells Violations Noted The temperature has been rising and the ice is starting to melt. Contacted the property owner and D&M and requested that the pads and booms be checked and replaced as needed.
1115297231 2005-04-26 Production Wells Violations Noted D&M Contracting has been beack ons site and removed impacted material north and west of the old tank battery. They also removed the concrete vault. All excavated material was placed in a roll-off box for landfill disposal. Sorbent pads will still be necessary at several spots but a majority of the spill appears to have been remediated. I will continue to monitor the site.
1186535901 2005-02-17 Production Wells Violations Noted D&M Contracting is still on site remediating the spill. Most of the impacted soil has been removed and disposed at Countywide Landfill. Oil is still present in the soil and on the ponded water. D&M is using a combination of sorbent boom, pads, peat and vacuum truck to recover oil. Weather conditions will prolong the recovery time. The siphon dam located at the wetland outfall is in good condition and does not appear to have been exposed to much oil.
1255098511 2004-03-11 Production Wells Violations Noted Status check inspection - a valve leak at the wellhead resulted in a crude oil spray that impacted an area 15 feet by 2 feet west of the wellhead. The impacted area needs to be remediated.
1573764842 2005-08-11 Production Wells Violations Noted There has been no recent site remediation. The corn planted in a portion of the affected area is slightly stunted but is growing. The affected area near the old tank battery is continuing to remediate naturally. There was no visible sheen on the ponded water. The NOV will remain open as long as natural remediation is occurring.
1831117088 2005-03-03 Production Wells Violations Noted D&M continues to provide spill remediation services at this spill site. Remaining activity includes oil recovery in the wetland, final soil removal and remediation and the dismantling of the original tank battery. A new 210 bbl tank has been placed near the wellhead.
1860213064 2005-06-23 Production Wells Violations Noted The majority of spill clean-up activity has occurred. There is still some impact in the dense vegetation north and west of the old tank battery which is remediating naturally. Will monitor the site through the summer.
