1411671286 2014-04-28 Production Wells No Violations I received a call from Brian Shpakoff (Atlas/Viking) on Sunday (4/27/14) about a flow line leak they had discovered on this location. I was on site this morning with Brian, Sanford Nisly, and contracted personnel to inspect remedial operations. Approximately 3-5 barrels of oil had leaked from the flow line between the wellhead and tank battery. The affected area is located in an old reclaimed strip mine with an approximate area of 100 X 30. The contractors have constructed 3 trenches which lead to a large siphon dam where they will collect any leaching product. The affected topsoil is being scraped and will be hauled to Kimble Landfill in Dover, Ohio. They will install booms and pads and monitor the location for the next week as the rain will flush out any residual oil into the siphon dam. Once the site is clean they will reclaim and seed the affected area. I will monitor progress.
1553623230 2019-03-26 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 1-1/2 tubing and rods and operating with the aid of an electric powered pumping unit. The wellhead and unit are fenced and locked and the required ID is posted at the wellhead. The wellhead is currently covered in oil and there is fresh absorbent material on the ground around the wellhead. The top of the polished rod liner is tagging and depressing the top of the stuffing box on the down stroke about 1/2. It also appears that the back pressure regulator on the tubing flow line is leaking at the set screw. I called Rusty Morrison (M&R) and informed him of my findings. He said he is going to raise the rod liner, repack the set screw, and clean up around the wellhead. This well produces to a diked tank battery to the north near the lease road entrance and is shared with the Newbury Investment #2 (PN#21845). The battery contains one (1) 210 barrel tank and a shared horizontal separator. There is a domestic tap on the sales line and the chart indicates a differential production pressure of 50psi. The required ID is posted on the tank. The issue at the wellhead is not currently a violation and I will check back next week to see if repairs have been made.
1693498339 2014-09-26 Production Wells No Violations Follow-up to a flow line leak that occurred on 5/20/14. The topsoil is stockpiled in a shallow, lined pit, and is covered. The soil is bio-remediating with the aid of lime and peat. I looked under the liner and the soil looks very good. The soil will be re-evaluated this spring. No violations.
1925538219 2014-05-21 Production Wells No Violations I received a call from Brian Shpakoff (Atlas) on 5/20/14 about an oil release at this location. He told me that the flow line between the well and the tank battery failed in the same spot that caused an oil release on 4/28/14 and that 2-3 barrels of oil had seeped to the surface. He asked for permission to remediate the soil on site as they had just replaced and seeded 140 tons of fill dirt. I told him this would be acceptable and discussed the proper remediation procedures. I was on location today to inspect the site. The stockpiled topsoil was properly covered and contained. I will continue to monitor the location.
