1058818213 2003-07-21 Production Wells Violations Noted On Saturday, June19, 2003, I received a call from Mike Loper, contract pumper for GE notifying me that an oil and/or brine spill had occurred on the Sicuro lease. During the previous week, the old 210 bbl stock tank had been replaced and 50 bbl brine tank had been removed. The impacted stone inside of the containment dike was also removed. The contractor was D&M Construction. During this activity, an 8 clay drain tile was discovered under the tank battery. Mrs. Sicuro indicated that a large portion of the property had been tiled by previous owners. The location of the tiles are unknown.

This spill was approximately 500 feet north of the tank battery and appeared to originate from the 8 clay drain tile. According to Mr. Loper the product appeared to be weathered and likely came from the old tanks. Apparently, the recent activity mobilized the product which surfaced down gradient, where the tile was blocked by sediment. Initial action on Saturday included laying sorbent pads and booms, exposing the clay tile at the spill, vacuuming free product and constructing a siphon dam.

On Monday July 21, I met on site with Aaron Gula (GE), Mike Loper, Dave Miller (D&M) and a D&M work crew to inspect the site and to discuss remediation. Additional booms were placed down gradient, used pads and booms were replaced and the siphon dam was fortified. A heavy rainstorm postponed exposing the tile from the spill area back to the tank battery. A visual inspection of a downstream pond and creek did not indicate any impact. Digging will resume as soon as the weather permits. Any impacted soil will be placed into a roll off box and disposed.

1201710043 2003-07-22 Production Wells No Violations Heavy rains had washed out the siphon dam which will be replaced. Work on removing the impacted tile will begin as soon as weather permits.
1231234840 2003-07-24 Production Wells No Violations Spill remediation continuing. The eight inch drain tile under he tank battery has been disconnected and plugged off with clay and cement to prevent access on either side of the tank battery. A sewer jet has been scheduled for tomorrow to clean the eight inch tile north of the tank battery to the release point in the pasture.
