0061193258 2006-10-25 Production Wells Violations Noted Investigate gas leak complaint at Peck Road north of Newton Falls Road. The sales line belongs to Equity Oil & Gas. I met on site with pumper Dennis Kingsbury. Wade Pol Contracting will be on site first thing in the morning to repair the pipeline.
0399864414 2009-07-07 Production Wells No Violations This inspection was completed at the request of Chris Lewis (330-297-9538), a nearby property owner in the Springtree allotment. There were no violations at this time. I asked the contract pumper to install additional signage on the fence surrounding the wellhead and to lock the gates on both the wellhead and the tank battery.
0846843074 2015-08-16 Administrative Inspection No Violations I received a call from the Ravenna Fire Department at 9:56am concerning natural gas being vented from the tank at this location. Equity has recently addressed these same issues at this location and I called the pumper, Jim Gove, and left him a message. Jim returned my call at 10:14am and I asked him to shut the well in immediately until the issue could be resolved. Jim called me back at 10:52am and told me the well was shut-in and that the dump valve in the separator was stuck open. I told him the well could not be placed back into production until the valve is either properly fixed or replaced. He will notify me when the proper repairs are complete. I will inspect the location tomorrow.
1135595363 2006-10-26 Production Wells No Violations Wade Pol Contracting was on site to expose and repair the salesline. The 1-1/2 line was clamped and the hole was backfilled. The violation is corrected.
1434395821 2015-05-06 UrbanProduction Wells Violations Noted Follow-up to complaint #5024. I met with Captain Mark Chappel & Chief Geoff Cleveland (Ravenna Fire) on location to inspect odor issues that they have been receiving concerning this location. I inspected the tank battery and found the dump valve was stuck open and gas was venting to the 140 barrel tank and out the open lid on top of the tank. This tank battery is located 30 from a tennis court and 75 from a condominium complex. The tank battery is fenced and the man door is not locked as required. The battery contains a horizontal separator and a 140 barrel tank. There is a 50 barrel poly tank that is not in use and the top of the tank is crushed, open, and 1/3 full of leaves and dark liquid. The tank battery has no dike and there is no ID posted on the tank or on the fencing. Also, the tank is not equipped with a charcoal filter or lightning arrestor as required in urbanized areas. The wellhead is located 500 to the northwest and operates with the aid of a plunger lift system. The wellhead is fenced and the man door is not locked. The required ID is not posted at the wellhead. During my inspection, the well cycled, and large amounts of gas were venting from the tank hatch. I could smell the gas/oil odors from the location of the wellhead. I called pumper Jeff Gove and Alaine King (Equity) to notify them of the safety and compliance issues at the location. Jeff arrived on location within 30 minutes and I had him shut the well down. I told him that the well could not be placed back into production until the above compliance issues were resolved. I will check back on progress.
1443622019 2015-08-17 UrbanProduction Wells No Violations I was on location to check the status of this well after receiving a gas release report from the Ravenna Fire Department on 8/16/2015. My inspection found the well shut-in as requested. Gates at the tank battery and wellhead were locked. The gas release caused a small amount of produced fluid to run down the side of the tank and there was a minor amount of staining on the gravel at the base of the tank. I will continue to monitor the location until the dump valve is either properly repaired or replaced at which time Equity may place the well back in production.

1741539198 2018-04-19 UrbanProduction Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead fenced and locked. The well is equipped with 1.5 tubing and operating with the aid of a plunger lift system. Gauges indicate a pressure of 130psi on the tubing and 620psi on the 4.5 production casing. This well produces to a diked and fenced tank battery to the south and contains one (1) 140 barrel tank and a horizontal separator. The tank stack is equipped with a self-contained charcoal filter and a low pressure relief valve. The battery is locked and the required ID is posted at the wellhead and at the tank battery. No violations.
1786771749 2015-05-08 UrbanProduction Wells No Violations Follow-up to Complaint #5024 and Compliance Notice #1434395821. The required ID has been posted at the wellhead and tank battery. Both of the access gates are locked as required and the well is shut-in. The remainder of the required work needs to be completed prior to placing the well back in production. I will check back later on progress.

The required ID has been posted at the wellhead and tank battery and both of the gates are locked. A containment dike has been built around the 140 barrel tank. The tank lid is secure and a charcoal filter with a low pressure relief valve has been installed on the vent stack. The tank is now equipped with a lightning arrestor and the pop-off valve on the separator has been plumbed into the sales line. I spoke with pumper, Jeff Gove, and informed him that the well may be placed back in production. He is currently working on getting the damaged poly tank removed from the separator and it is disconnected and not in use. The complaint and associated compliance issues have been addressed and resolved. This complaint and compliance notice is closed. No violations.
