0036379011 2002-08-14 Production Wells No Violations See Case 02-035. 8-13-02 Received complaint from Elizabeth Huzl (216-651-5885) regarding a pipeline leak on her property located at 5903 Streeter Rd. in Freedom Twp., Portage Co. 8-14-02 Inspected the site; contacted John Firko (Power Gas); met with Mr. Knipper (330-274-2476), a neighbor at 5934 Streeter Rd; talked with Robert Martin, Knippers grandson. The line leak was already repaired. COMPLAINT IS RESOLVED. 8-15-02 Informed Mrs. Huzl that the line was repaired.

A second spill source has emerged approximately 100 feet north of the original. Power Gas was notified and said that they would have someone repair the leak today. The affected area at the source, the road from the Knipper tank battery and the staging area near the Knipper tank battery will all need to be reclaimed.

0418930467 2004-02-19 Production Wells Violations Noted A three inch sales line failed and released oil into a nearby creek. OEPA was notified and issued the incident #0402-67-0648. Jim Erwin responded. The line is located approximtely 500 feet east of the Knipper well. The release was estimated at five bbls by Power Gas. The spill impacted approximately 700 feet of stream bed. Power Gas personnel constructed a retention dam near the spill source; a siphon dam downstream ahead of the spill; and placed sorbent pads and booms along the stream bed. The pads and booms will be replaced and needed and the impacted soil will be removed. The site will be monitored regularly for compliance.

The soil is being transferred in rolloff boxes to Petrocell in Lodi for disposal. On this date, two rolloffs were filled and removed. As long as the work continues in a timely manner, no additional enforcement action is recommended.

According to Power Gas personnel, the large trackhoe will be on site on 3/16 to start removal operations. The soil will be placed into a lined rolloff box, tarped and transported to Petrocell in Lodi, Ohio for disposal.

1433473990 2005-09-20 Production Wells Violations Noted While checking the status of this well, I met a Power Gas employee cleaning up a spill around the wellhead resulting from another stuffing box failure. He had no knowledge of any plans to replace the leaking 210 bbl tank. I told him what I wanted as far as cleaning up this latest spill and then showed him the gas leak near the Jones #1 tank battery.
1687141173 2004-03-09 Production Wells Violations Noted Little has occurred since my last inspection on February 26. The pads and booms located in the stream channel need to be replaced. The siphon dam has been breached and will not last much longer. The impacted soil at the release point has not been removed and disposed as requested. I contacted John Firko who stated that they will have a skid box on site Monday March 15 to move the soil from the release point to the lease road. If this commitment is not met, I will contact OEPA and recommend that fines be issued for noncompliance.
