0046593202 2014-11-14 UrbanProduction Wells No Violations Follow-up to Complaint #4639. Enervest has locked and secured the gates at the tank battery. The lease road shares the driveway apron with the complainants driveway. The wellhead and tank battery are not located on the complainants property. The well is still idle and has not produced commercially since being shut-in on 12/19/2012 due to a bad sales line. I will speak with Enervest about placing the well back in production or properly plugging and abandoning the well. This complaint continues.
0121471989 2017-09-07 Plug / Plug Back No Violations I was on location to witness plugging operations on this Clinton Sandstone well. Chipco, LLC was the plugging and cement contractor, Enervest and J. Hawk were supplying brine and freshwater for operations. Enervest personnel were on location to oversee plugging operations. A circulating/blow pit was dug and lined on the east side of the wellhead. The pressure on the 4.5 production casing had recovered to 700psi this morning after blowing the well down for several hours yesterday (6/6/17). Chipco attempted to recover the plunger from the 1.5 tubing by blowing it down for 0.5 hours to the pit and no recovery was made. They hooked up to the tubing to determine if the plunger was, in fact, in the tubing. They pumped 10.5 barrels of water down the tubing and the plunger landed and plugged off the tubing. Chipco then hooked up to the 4.5 casing and pumped 65 barrels of brine to kill the wellhead pressure. They then pulled 143 joints of 1.5 tubing (wet) from the hole. The wellhead was then fitted with a 4 x 2 reducer and ball valve and Chipco rigged up to the wellhead. They then mixed and pumped a 6.3 barrel (30 sack) plug with 40lbs of cottonseed hulls and bullheaded the plug to 3980 with 64 barrels of water. The plug landed at 800psi and the wellhead was shut-in. Operations will continue tomorrow (9/8/17). No violations.
0450120235 2017-09-12 Plug / Plug Back No Violations I was on location to witness drill-out operations on this recently plugged Clinton Sandstone well. Chipco was rigged up and drilling cement inside the 8.625 surface casing with a 7.875, tri-cone, button bit. They used two (2) 6 drill collars for weight. The hole was circulating good during the drill-out and they were at a depth of 50 when I left location. Tim McConaha (Enervest) called me at 4:12pm and told me that they had hit metal at a depth of 60 and thought it was likely an offset casing collar. He said they would need to get a smaller bit and collars to continue the drill-out. I will meet with Enervest on location tomorrow (9/13/17). No violations.

There is a bulldozer parked just past the tank battery and the location around the wellhead has been cleared, bladed, and graded. I will check back later to see if Chipco has moved in. No violations.
1316421538 2017-09-19 Plug / Plug Back No Violations I was on location to check the top plug in this plugged and abandoned well. The top of the plug was five (5) feet below grade and solid. I found no indication that the plug was leaking. Chipco is rigging down and will cut the 8.625 casing off and attach the ID plate. The pit used for the drill-out has been emptied and the pit is being closed. No violations.
1397676647 2014-03-20 UrbanProduction Wells No Violations Follow up to idle well complaint #4639. My inspection found the well shut in and equipped with a plunger lift system. A domestic hook up off the well head was closed and not supplying house gas. I inspected the tank battery and found that it was diked and fenced. The fence was in disrepair and the gate was unsecured. The battery contains 1-100 barrel tank, a horizontal separator, and 2-brine vaults. The inside of the battery is overgrown with briars and weeds and the ID is posted on the fence. A note inside the separator indicates that the sales line was shut off on 12/19/2012. Production has not been reported for the last 2 consecutive reporting periods and the well is idle. I will speak with Enervest and the landowner about my findings.
1504718293 2017-09-05 Plug / Plug Back No Violations I was on location with Doug (Enervest), and Chipco, LLC to prepare for plugging operations on this Clinton Sandstone well. There was 700psi on the 4.5 production casing and 600psi on the 1.5 production tubing. Chipco spotted the rig and dug up the 8.625 wellhead. The ports on the 8.625 surface casing were open and there was no pressure on the annulus indicating 4.5 casing integrity. The port on the 4.5 casing was fitted with a 2 line and ball valve. I contacted the Rootstown Fire Department to inform them of the plugging operations and that the pressure on the well would be blown down to the atmosphere. Chipco opened the valve and began bleeding off the pressure. After one (1) hour there was 500psi on the 4.5 casing. After another hour of venting the pressure bled down to 120psi and began misting fluid. At this point I requested that the valve be closed and a new plan be developed. It was decided that a pit will be dug and lined in the morning for further bow-down operations. Chipco will bring their pump truck and Enervest will have a brine truck on location to kill the well if it will not blow down to safely trip the 1.5 tubing out of the hole. The wellhead was prepped for operations tomorrow (9/6/17) and packed in and secured. No violations.
1505745584 2017-09-18 Plug / Plug Back No Violations I was on location to re-plug this well that was drilled out last week. Chipco spotted 6.5lb/ft., EUE tubing at the top of the Big Lime plug at a depth of 2244. They rigged up to the tubing, broke circulation and mixed and pumped 18.9 barrels (90 sacks) of cement slurry. They displaced the cement to 1944 with 11 barrels of water. Chipco pulled 57 joints of tubing to a depth of 506, rigged up, and broke circulation. They then mixed and pumped cement (23.1bbls) until good returns were achieved at the surface. The tubing was pulled and the hole was topped off. I will check the top plug tomorrow (9/19/17) for integrity. No violations. See plugging report for more details on this job.

1618525221 2015-08-05 UrbanProduction Wells No Violations Follow-Up to idle well Complaint #4639. My inspection today found that the well remains shut-in and idle. Production records indicate the last commercial production from this well was in the reporting year 2010 of 173mcf of natural gas. In 2011, Enervest reported 80mcf of natural gas production from this well. In the following three (3) reporting periods (2012-2014), Enervest has reported zero (0) barrels of oil and zero (0) mcf of natural gas production from this well. Writing on the chart box indicates that the sales line from this well was shut off on 12-19-2012. Conversations with Enervest field personnel indicate that the sales line has lost integrity and will likely not be repaired to place the well back into production. Based on my findings, Enervest Operating, LLC will be given 180 days to 1) obtain a plugging permit as required by Chapter 1509:13 of the Ohio Revised Code and 2) properly plug and abandon the well in accordance with Chapter 1501:9-11 of the Ohio Administrative Code. Failure to comply with this notice may result in further administrative action by the Division.
1627963617 2017-09-11 Plug / Plug Back No Violations I was on location to check the status of the top plug in this well that was set on Friday (9/8/17). My inspection found the top plug leaking gas (bubbling) between the perimeter of the plug and the inside of the 8.625 surface casing. The plug was two (2) feet below grade and solid. I tested the air inside the surface casing with a Honeywell PhD6 gas meter and found 50ppm of carbon monoxide (CO), 0.4 of the lower explosive limit (LEL), and 0.5ppm methane (CH4). Chipco was on location and I spoke with Tim McConaha (Enervest). They will bring in the necessary equipment to begin drilling out the top two (2) plugs tomorrow to remediate the leak. Chipco rebuilt the wellhead on the 8.625 surface casing and shut the well in for the evening. No violations.
1630769393 2017-09-14 Plug / Plug Back No Violations I was on location to witness drill-out operations on this recently plugged well. Chipco was drilling with a 4.875 tri-cone, tooth bit, six (6) 3 drill collars and 6.5lb/ft., 2.875, EUE tubing. They are currently drilling cement at a depth of 193 and should have been out of the bottom of the top plug at this point. I spoke with Inspector John Fleming later in the day, who was covering the drill-out, and he informed me that Chipco was still drilling cement at a depth of 233 and are shutting down for the day. They will continue the drill-out tomorrow (9/15/17) under the supervision of Inspector John Fleming.
1759082888 2017-09-13 Plug / Plug Back No Violations I was on location with Inspectors Bob Roush and John Fleming to meet with Enervest personnel and discuss the drill-out and re-plugging of the well going forward. We examined the metal cuttings that were recovered, but they were to fine to determine the source of the metal. It was decided to drill with a smaller bit through the top two (2) plugs and tag up and dress up the top of the Big Lime plug. Energy Services was delivering six (6) 3 drill collars and the proper change-over subs to location during the meeting. Chipco will continue with the drill-out tomorrow morning (9/14/17). No violations.
1989663986 2017-09-08 Plug / Plug Back No Violations I was on location to continue with plugging operations on this Clinton Sandstone well. The valve on the 4.5 casing was opened and the well was static. The fluid level in the 4.5 casing was 10 down and indicated that the bottom plug held. The casing was ripped at 2412 and plugs were set to isolate the top of the Big Lime, the Berea Sandstone/Ohio Shale interval, and the Glacial Deposits. I will check the top plug on Monday (9/11/17). See plugging report for more details on this job. No violations.
