0529357335 2018-06-25 Final Restoration No Violations Status check inspection for Final Restoration. My inspection found that the 100 barrel tank has been removed. The area of the former tank battery has been graded and seeded and the seedbed is beginning to establish. The area of the former wellhead has been incorporated into the corn field, has been planted by the farmer, and corn is beginning to grow. The lease road is access to the farm field and the lease road for the Kirkbride #1 (PN#21035) & the Leroy Myers #2 (PN#21360) and does not require removal at this time. I have received the executed Form 5 (Landowner Waiver) waiving the removal of the lease road and flow line, and allowing the crushing and burial of the concrete brine vault. Final Restoration is approved. No violations.
0598049048 2018-07-30 Non Well No Violations Status check inspection after approving Final Restoration of this Clinton Sandstone well that was plugged and abandoned on 5/29/18. My inspection found that vegetation is well established at the tank battery area and corn is growing in the area of the former wellhead. No violations.
0786353510 2018-05-25 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. Hagen Well Service (HWS) ran inside the 4.5 production casing and tagged the top of the bottom plug at a depth of 4350. HWS parted the 4.5 production casing at a depth of 3200 and they recovered 73 joints of 9.5lb/ft. casing. They are going to run tubing back into the hole and shut down for the weekend. See plugging report for more details on this job. No violations.
0800620313 2018-05-24 Plug / Plug Back No Violations I was on location to witness the plugging and abandonment of this Clinton Sandstone well. Hagen Well Service spotted 1.5 tubing at a depth of 4707 and Petroset Cementing rigged up to the tubing. They cleared the tubing and then mixed and pumped 7.3 barrels (30 sacks) of Class A cement slurry. The plug was spotted inside the 4.5 production casing, across the Clinton Sandstone, from 4707 to 4257. Hagen Well Service began pulling the tubing from the well and they will shut down for the day when it is recovered. See plugging report for more details on this job. No violations.

Hagen Well Service and Petroset Cementing spotted cement plugs through 1.5 (ID) tubing to cover and isolate the top of the Big Lime, the Berea Sandstone/Ohio Shale interval, and the Glacial Deposits. 1397496493 2018-05-25 Plug / Plug Back No Violations I was on location to witness plugging operations on this Clinton Sandstone well. Steve Hagen Well Service tagged the top of the bottom plug at 4350. They ran in the hole with the knife tool and parted the 4.5 production casing at a depth of 3200. The casing was recovered (73 joints of 9.5lb/ft., Range-3) and they ran 94 joints of 1.5 tubing into the hole to a depth of 2690, packed in the well, and shut down for the Holiday weekend.

I left a message with the company representative for the plug job, Paul Hesse, for an update on when plugging operations will begin. Paul called me back and notified me that operation have been delayed for at least one (1) week. No violations.
1527271386 2018-05-24 Plug / Plug Back No Violations I was on location to witness plugging operations on this Clinton Sandstone well. Steve Hagen Well Service spotted 1.5 tubing at 4707 and Petroset Cementing rigged up and spotted a 30 sack plug (450) to cover and isolate the Clinton Sandstone. The tubing was pulled and the well was packed in for the day. See plugging report for more details on this job. No violations.
1527684829 2018-05-30 Plug / Plug Back No Violations I was on location to check the integrity of the top plug set in this well on 5/29/18. The plug has dropped and I tagged the top of the plug with a reel tape and weight at 39 below the top of the 8.625 surface casing. The reel tape indicated 7 of fluid on top of the plug. I found no evidence that the plug was leaking or had lost integrity. I spoke with Paul Hesse (Company man for the plug job), and he is going to top the hole off this afternoon with his cement truck. No violations.
1528126571 2018-06-04 Plug / Plug Back No Violations Status check inspection of this recently plugged and abandoned Clinton Sandstone well. The area of the wellhead has been filled in and graded. The separator has been removed from the tank battery and a 100 barrel tank and brine vault remain. I sent an e-mail to Mark Sparr (J.R Smail) to address his questions on the procedure to remediate the brine vault, prior to reclamation, that the landowner has approved to be crushed and buried in place. Reclamation of the vault should take place later this week. No violations.
1528391829 2018-06-07 Plug / Plug Back No Violations Status check inspection of the cleaning, crushing, and burial of the brine vault at this recently plugged and abandoned well. The landowner, Glen Myers, has authorized the disposal of this tank on the Form 7 Landowner Waiver. Scott Miller Contracting (SMC), J.Hawk, and personnel from J.R Smail were on location to complete the job. The lid was removed and SMC steamed the sides, lid, and bottom of the tank while J.Hawks water truck collected the fluids. The solids and debris were removed for disposal and the tank was steamed again with 30 barrels of hot water (180 degrees). All the fluids were removed and three (3) bags of cement were used to solidify the bottom of the tank. The perimeter of the tank was excavated below the base and there was no sign of contamination. The vault and lid were broken up with an excavator and the debris was covered with garden lime prior to burial 5-6 below grade. There were no violations at this time.
1630272360 2018-05-30 Plug / Plug Back No Violations Follow-up to an inspection conducted this morning that found the top plug 39 below the top of the 8.625 surface casing. My inspection found that the hole has been topped off to 4 below the top of the casing and there are no observable leaks. The casing may be cut off and the ID plate attached. No violations.
1869996958 2018-06-13 Final Restoration No Violations Status check inspection on progress towards Final Restoration status. The stone from the interior of the former battery has been removed and the dike walls have been graded out and blended with the current landscape. The 100 barrel tank and ladder have not been removed and there is a trac-hoe and bull dozer on location. There are no violations at this time.
1939689015 2003-12-02 Production Wells No Violations Status check inspection - the well ID sign has faded and the well name and permit number need to be re-lettered. I contacted Orion with this request.
2015815350 2017-11-21 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 1.5 tubing and a plunger lift system. The well is shut-in and there is a domestic tap and meter on the 4.5 production casing that is supplying house gas. The electronic controls have been removed from the box on the motor valve. The last commercial production reported for this well is for the 2013 reporting period. 35Mcf of natural gas was reported for the 2014 reporting period and 0Mcf of natural gas and 0 barrels of oil were reported for the 2015 & 2016 reporting periods. There is a 5 diameter hole in the ground next to the 4.5 production casing and natural gas is vigorously bubbling through fluid below grade in the hole. Either the production casing has a hole in it or the buried 8.625 surface casing is pressured up and leaking to surface. The tank battery is diked and contains one (1) 100 barrel tank, a concrete brine vault, and a horizontal separator. There is no chart currently in the meter box. ID is posted at the wellhead and the tank battery. I called and left a message with Mark Sparr (JR Smail) about the leak at the wellhead and will wait for a return phone call. The leak does not currently pose a threat to life and limb or the environment, but needs to be addressed in a timely manner. I spoke with Mark later and he said that their production supervisor will evaluate the cause of the leak. I also advised him to consider Temporary Inactive Status or plugging the well based on production records (no commercial production since 2013) and he will get back to me with a plan. I will follow-up next week.
