0200868426 2017-12-18 Final Restoration No Violations Status check inspection for Final Restoration of this plugged and abandoned Clinton Sandstone well. The well was plugged on 10/19/2017 and final restoration is due by mid-January of 2018. This site is being quickly developed as a residential subdivision. My inspection found that all the surface production equipment has been removed from the location and the entire area has been regraded by construction operations. The lease road no longer exists and a Landowner Waiver (Form 5) will need to be submitted to leave the flow and sales (disconnected) lines buried for Final Restoration to be passed. No violations.
0460712876 2017-10-20 Plug / Plug Back No Violations I was on location to check the top plug in this Clinton Sandstone well. My inspection found the top plug solid and set at the 8.625 surface casing port. I found no evidence of integrity issues or leakage. The service rig has been moved to the Beljon #2 (PN#4378). The wellhead will be cut off next week and fitted with the ID plate. No violations.

1394209121 2014-03-06 UrbanProduction Wells Violations Noted I conducted a status check inspection of this Clinton well. My inspection found the well operating with the aid of an electric pumping unit. The pumping unit was fenced but the access gate was not locked and there was a domestic hook up off of the well. The tank battery was located adjacent to the wellhead. The battery was fenced, diked, and contained 1-210 barrel tank, a 100 barrel tank, and a brine vault. Two vertical separators and a meter run were inside the battery on the north side of the tanks. The gate to the tank battery was not locked and the pad lock and chain were hanging on the gate. The ID sign posted at the tank battery indicates Waterco Inc. as the owner/operator of the well and lists the wrong emergency contact phone number which is in violation of OAC 1501:9-9-05(A)(10). David A Waldron & Associates will, within 1 week, post the proper identification indicating the proper ownership, emergency contact phone numbers, and other information as required by law.
1508504703 2017-10-19 Plug / Plug Back No Violations I was on location to witness plugging operations on this Clinton Sandstone well. Moore Well Services was the plugging contractor, Petroset Cementing was the cementing contractor, and Appalachian Well Surveys provided wireline service. Plugs were emplaced across the Clinton Sandstone, the top of the Big Lime, the Berea Sandstone/Ohio Shale interval, and the Glacial Deposits. A cast iron bridge plug (CIBP) was set inside the 8.625 surface casing at 225. A total of 2545 (76 joints) of 4.5 10.5lb/ft., Range-2 production casing was recovered during the job. See plugging report for more details on this job. No violations.
1523284154 2018-04-09 Final Restoration No Violations Status check inspection for Final Restoration of this Clinton Sandstone well that was plugged and abandoned on 10/19/2017. My inspection today found that the area of the former wellhead has been incorporated into Phase 1 of the Beljon Estates subdivision and has been overprinted by development. All equipment has been removed and the lease road no longer exists. The sales line/Dominion tie-in point and metering station to the north has been removed and the tie-in was disconnected by Dominion prior to plugging operations. An executed Landowner Waiver (Form 5) has been submitted by Pulte Homes of Ohio waiving the removal of any flow lines that have been abandoned on the property. Final Restoration is approved.
1933382434 2014-03-20 UrbanProduction Wells No Violations I was on location to follow up on Compliance Notice #1394209121 to post the required ID sign. The sign had not yet been posted. I spoke with Jerry Reeves (DA Waldron) and he informed me that the sign was ready but the pumper has been sick all week. He said he will get it posted tomorrow. This violation continues.
