0168483547 2005-03-11 Production Wells Violations Noted A crew from Arvilla Oilfield Service was on site to remediate the spill. The spilled liquids in the dike and in the brine tank were collected by Pander Trucking. The tank was removed and the impacted soil was removed and transported to Arvillas yard.
0194933460 2003-04-01 Production Wells Violations Noted Status check inspection - the well operation is causing pollution around the 100 bbl stock tank. The south half of the dike area is impacted with oil from tank overfills. The separator is also leaking. The impacted area inside of the dike needs to be removed and disposed of properly. The wellhead area is clean. I contacted Buzz West (330-364-8155) with Orion to discuss the condition of this tank battery.

0812983725 2005-03-10 Production Wells Violations Noted During a status check inspection, a spill was found around the 50 bbl brine tank. Approximately one bbl of oil and brine had been released from the tank into the secondary containment dike. Unfortunately, the containment dike had been cut, which permitted the release of product out of the dike. I also found that the valve on the bottom stock tank drain was leaking. I contacted Buzz West and requested that the brine tank be removed and the spill remediated and that the stock tank valve be replaced. He said that a crew would be on site the next morning.

1181526916 2017-10-06 Production Wells No Violations I was on location to follow-up on Compliance Notice #1503504129. My inspection found that the soil stockpiled for removal remains on location and is tarped and contained. A 140 barrel tank has been spotted and plumbed in to the dump line at the separator. I found the well pumping and the stuffing box was leaking oil and gas out of the top of the gland cap and around the polished rod. There were 5-6 gallons of oil on the ground around the wellhead and had migrated west about 10 and into a depression. I placed absorbent pads in the depression and I called Ted Lawver (Pin Oak) to notify him of the issue. A pumper arrived within fifteen minutes, shut the pumping unit off, and went to retrieve more pads and Peat-Sorb to soak up the oil. The oil was soaked up with pads and the pads were placed in a five (5) gallon bucket for disposal. I will check back next week for any impacts that may need addressed.

1345559941 2017-10-20 Production Wells No Violations Follow-up to Compliance Notice #1503504129. My inspection found the tank battery has been rebuilt. The dump line from the separator has been plumbed to a rebuilt 140 barrel tank. The dike walls have been rebuilt around the perimeter of the tank battery and the contaminated material that was stockpiled in the lower containment area has been removed. The lower containment area as well as the leaking 50 barrel tank have been removed. All the affected material has been removed and disposed of at Waste Managements American Landfill. This violation has been resolved.

1503504129 2017-08-22 Production Wells Violations Noted I conducted a status check inspection of this Clinton Sandstone well. My inspection found the well equipped with a natural gas powered pumping unit. The well produces to a diked tank battery to the east and contains one (1) 100 barrel tank, a horizontal separator, and a meter run. There is a 50 barrel steel tank on the south side of the battery with its own containment. An area measuring 15x18 around and under the 100 barrel tank has been impacted by an oil release and needs to be remediated. The heater tube on the 100 barrel tank looks to have been repaired with putty and the open end is covered with duct tape. The tank has also leaked oil from a bull plug, six inches from the top of the tank, and the tank is stained with oil. The 50 barrel tank is rusted and there is oil staining and water around the base of the tank. I ran a high rang Quantab in some standing water at the base of the tank. The Quantab read 7.8 units and corresponds to a chloride concentration of 4300ppm and indicates the tank or connection is leaking brine. The dike walls around both tanks have been cut to allow water to drain out of containment and could not contain a release. I spoke with Ted Lawver (Pin Oak Operations Manager) about the issues McKnight #2 and this well. He is going to meet me on location tomorrow (8/23/17) to go over what needs to be done to remediate these Compliance issues. Pin Oak shall, within one (1) week, undertake the following remedial operations: 1) pull both tanks and determine if they need to be replaced or repaired, 2) excavate and remove any brine and oil saturated material for disposal, 3) remediate any brine affected areas with lime, 4) repair the cuts in the dikes and build up the secondary containment walls where necessary.
1506106254 2017-09-22 Production Wells No Violations Follow-up to Compliance Notice #1503504129. Clean-up operations were undertaken by Lakeside Oilfield, at the tank battery, on 9/22/17 under the supervision of Inspector John Fleming. The 100 barrel tank was steamed and removed from location. My inspection today found that the contaminated material has been stockpiled in a diked containment area and covered with a plastic liner. Clean stone has been placed in the battery and the 50 barrel tank has been placed on posts within the battery. I spoke with Ron Hammen (Lakeside) and they will be continuing operations on Monday. Emergency Operations Coordinator Fred Romeo stopped by the location to check on progress and I showed him what had been done to this point. This violation continues.
1587008220 2018-05-09 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 1.5 tubing and rods and operating with the aid of a natural gas powered pumping unit. This well produces to a diked tank battery to the east that was remediated in the fall of 2017 and the battery is in good condition. The battery contains one (1) 140 barrel tank, a horizontal separator, and an associated meter run. A gauge at the separator indicates a pressure of 220psi and the required ID is posted on the tank. No violations.
