0878694123 2014-09-17 UrbanProduction Wells No Violations Follow up to Complaint #4799 concerning a possible leaking well. I left a message with Mr. Svetlak on 9/15/14 about the complaint call. My inspection today found that the proper ID is not posted at the wellhead or tank battery and none of the gates are secured with locks as required. The annulus between the 7 surface casing and the 4.5 production casing has minor bubbling. A gauge indicates 200psi on the production casing. The door to the meter shack is locked and inaccessible and the valve on the sales line into the meter shack has been closed. Mr. Svetlak has not addressed any of the issues discussed during a closed door meeting at the Uniontown Field Office on 5/11/14. I will contact Mr. Svetlak again, and, if he is unresponsive, a compliance notice will be issued. This complaint continues.
1067552322 2013-10-11 UrbanProduction Wells No Violations I inspected this well to follow up on Chiefs Order #2013-136 issued on 9/25/2013. The operator, Stephen J. Svetlak, LLC, has failed to maintain proof of liability insuarnce as required by ORC 1509.07(A) and has been found in matierial and substatial violation for failing to maintain coverage. My inspection found that the well was still in production and is in direct violation of the Chiefs Order to immediately suspend all oil and gas operations in the State of Ohio and 1509.07[C] of the Ohio Revised Code. I documented that the well is producing into a Dominion/East Ohio Gas line and took pictures inside the meter run showing production and that the valves were open and selling natural gas. The chart was not labeled and was being maintained and changed out by Tri State Measurement. The operator has not reported production since taking ownership of the well in 2005. The required ID was not posted anywhere on location or on any of the surface equipment. None of the gates were secured with locks and the fencing on the north side of the tank battery was breached and falling down. The wellhead was equipped with a gauge indicating 125psi on the casing. The annulus between the surface and production casing strings was closed and visible bubbling was occurring around the wellhead. The tank battery was diked and contained 2-100 barrel stock tanks and a brine vault. The brine vault is no longer usable as the top is broken in half and is partially submerged by the 8-12 water contained within the dike. The dump line is disconnected and the vault is completely full of water. I ran a low level Quantab and did not find elevated levels of chlorides. The well is producing through a vertical separator to the west of the wellhead and is overgrown with weeds.
1217050662 2018-10-01 UrbanProduction Wells No Violations I inspected this well to follow-up on a request made to Bocor Holdings to dig up and expose the annular valve on this wellhead and check for pressure. Jim Paumier (Field Supervisor) notified me on Friday (9/28/18) that they had exposed the annular valve and blew down pressure in two (2) - three (3) minutes. He said there was a minor amount of gas flowing after it blew down and he is going to plumb the annulus to the flow line. My inspection today found that annulus between the 7 surface casing and 4.5 production casing has been plumbed to a valve on top of the casing access/swab port. A gauge on the casing indicates a pressure of 160psi and there was no communication reported between the annulus and production casing. No violations.
1400995506 2015-07-27 UrbanProduction Wells No Violations Follow-up to complaint #4799 concerning a possible leaking well. This well was sold by Stephen J. Svetlak, LLC to Bocor Holdings, LLC and the transfer was completed on 5/29/15. My inspection today found that the issues found at this location previously have been corrected. The proper ID signage is now posted at the well and the gates to the wellhead and tank battery are locked as required. The gas that was bubbling from the annulus between the 7 surface casing and 4.5 production casing (and was the source of the complaint) has been mitigated. The well is in production and I found no violations at this time. This complaint is resolved.
