0893209506 | 2016-01-14 | Production Wells | No Violations | Re-visit site. Photographic evidence that flooding in the area has caused some of the fluid in the well to have exited the casing and be deposited onto the ground surface, creating a mud ring around the casing. 7 photos collected. |

1181228621 | 2016-01-13 | Orphan | No Violations | Landowner interview and relayed to the landowner the steps involved in the plugging of this well. Informed landowner that she would be kept appised as to the progress of the case. |
1257805041 | 2016-01-13 | Production Wells | No Violations | Contacted Mrs. Hodgeman on this visit. She took me to the well site and I witnessed the fluid within the well bubbling with escaping gas. Mrs. Hodgemans main concern is the flooding in the area during the Spring season. I was unable to detect any readings with the Sperian Gas meter. Collected 6 photos and GPS readings. |
1265761832 | 2016-10-14 | Orphan | No Violations | My inspection found the well to be open and venting natural gas into the atmosphere. A weighted line was dropped into the open bore hole and no obstructions were encountered. The weighted line was lowered to 300 and then removed. The fluid level in the casing was at ground level. |
1419509214 | 2018-09-05 | Orphan | No Violations | Status check inspection of location construction. My inspection found Daley Excavating has installed the cellar (tin horn) around the wellhead and properly trenched in the silt fence on the east side of the location. Four (4) panels of chain link fencing were erected around the cellar for safety. I found no violations at this time. |
1452882639 | 2016-01-11 | Orphan | No Violations | Attempted to verify condition of well casing but it was submerged under water. |

1466090031 | 2016-06-16 | Orphan | No Violations | On site today to run weighted line downhole to determine free space depth. Sat down solid at +/- 632 feet. Encountered no obvious obstructions during the descent of the line; no problems extracting line from hole. |
1540309119 | 2018-10-23 | Orphan | No Violations | Status check inspection of drill-out and re-plugging operations. I was on location with Gene Chini, Kevin Perry, and Bishop Well Services. The well blew out yesterday (10/22/18) after drilling out a wood plug at 1400 and unloaded freshwater drilling mud to the steel tanks and sprayed the entire location. Bishop personnel were scraping up the remaining mud and cleaning off the mats. Operations were discussed moving forward. No violations. |
1540567395 | 2018-10-26 | Orphan | No Violations | I was on location this morning to check on operations at this orphan well. My inspection found that there is currently no activity on location. I spoke with Inspector Kevin Perry and he told me that they are currently at the 7 casing seat and have 200 of plug to drill before encountering the 4.5 production casing rip point. Operations will resume on Monday (10/29/18). I inspected the location and it is secured with chain link fencing. I did not observe any gas vapors discharging from the four (4) inch flow line to the steel pit. No violations. |
1541088768 | 2018-11-01 | Orphan | No Violations | Status check inspection of this orphan well location. There is currently no activity on this location. The access road is matted and the location is fenced and secured. I spoke with Bob Roush and he told me that two (2) plugs will be spotted tomorrow beginning at 8:00am. There were no violations at this time. |
1692322089 | 2018-10-22 | Orphan | No Violations | I received a call from Kevin Perry ( @ 1555 hrs. ) requesting that I inspect the Hodgeman #1 Orphan Well re-plugging project after well unloaded the mud system subsequent to drilling out a plug at approximately 1400. The plugging contractor was Gonzoil Inc. with the service rig supplied by Bishop Well Service. When I arrived on location ( @ 1630 hrs. ) the rig crew was re-loading the wellbore and the well was slightly gassing. I spoke with the rig operator ( Bruce ) who said, that after drilling approximately 200 of clay plug and wood fibers circulation was lost momentarily and the well unloaded the drilling fresh water drilling mud into the half round circulation tanks. The force of the well caused some of the mud to be spilled out of the tanks and onto the ground. Fred Romeo and Andrew Adgate ( Emergency Response ) where on location to assess the situation. The wellbore was re-loaded with drilling fluid and allowed to vent overnight. Doug Gonzales was requested to remain on location overnight to monitor the situation. |
