0002403996 2018-09-20 UrbanProduction Wells No Violations I inspected this well at the request of the Orphan Well program to check the pressure on the annulus between the 7 surface casing and the 4.5 production casing. I spoke with Field Supervisor Kyle Ahbel this morning and he is going to inform Joe Teppert of my request to expose and gauge the annular pressure. Joe called me back and told me that Josh Schrock had dug up the annulus and found that the annulus and casing pressure were equalized at a pressure of 490psi. He told me the well has been, and is currently, shut-in to replace the 100 barrel tank which has been completed. The equalized pressure indicates that the 4.5 production casing has lost integrity and is charging up the annulus with pressure. The well is currently equipped with 1.5 tubing and a plunger lift system. This well produces through a vertical separator to the east of the wellhead and to a diked tank battery to the west containing a refurbished, 100 barrel tank. It is determined at this time that the surface casing seat is over pressurized and exceeds the maximum allowable pressure of 180psi (.303 x 594). I called Joe Teppert and asked him to begin selling the pressure off of the well. The pressures will be monitored at the wellhead until the issue is corrected.
0268010291 2018-12-28 UrbanProduction Wells No Violations Follow-up to Compliance Notice #1650442205 issued for defective casing that resulted in over pressurization of 7 X 4.5 annulus that was found to be 490psi on 9/20/18. I inspected the location today and found that the annular pressure gauge is reading just over 160psi and is below the maximum allowable pressure of 180psi. The 1.5 gate valve below the plunger lubricator is closed and the valve on the 4.5 production casing port is open and keeping the pressure off of the 7 surface casing annulus. There is currently 12psi on the domestic supply line. The chart at the meter shack (MOA) shows line pressure fluctuating between 170psi and 150psi. I spoke with Enervest Southern District Superintendent Tim McConaha and he asked for an extension to plug the well by May 1, 2019. Tim told me that the well is currently supplying domestic gas to a rental property and they have no other source of gas for heat at this time. He said he is currently working with Dominion to get gas service for the property. A plugging permit was issued to Enervest on 12/7/2018 to plug and abandon this well. I have approved the extension until May 1, 2019 contingent that Enervest continue to monitor the annular pressure, and that it is not allowed to exceed the maximum allowable pressure of 180psi. This violation will continue until the well is properly plugged and abandoned.

There is currently 7psi on the domestic supply line. I spoke with Enervest Operations Superintendent Doug Beaghle on 10/22/18, and he proposed resetting the tubing on a packer, just above the perforations. I told him that this would be acceptable contingent on the annular pressure being remediated. I spoke with Operations Superintendent Tim McConaha on 11/6/18, and he told me that Enervest has decided that it would be more economically feasible to plug and abandon the well. He requested an extension to plug the well in the spring of 2019 when the weather conditions improve. I told him I would discuss the extension with DOGRM Northeast Regional Supervisor Tom Hill and let him know if the extension is approved. I will continue to monitor the annular pressure at this well for compliance.

1550071437 2019-02-13 UrbanProduction Wells No Violations Follow-up to Compliance Notice #1650442205 for defective casing. I inspected this well to check the annular pressure between the 7 surface casing and 4-1/2 production casing. The gate valve on the 1.5 tubing and plunger lift assembly is currently closed. The casing port valve is currently open to keep pressure off of the annulus and the annular gauge reads170psi and is below the maximum allowable pressure of 180psi. The domestic tap is continuing to supply house gas at a pressure of 7psi. The current line pressure at the meter shack (MOA) is between 160psi & 170psi. This well is scheduled to be plugged in the spring of 2019 and the violation will remain open until that time.

1650442205 2018-10-01 UrbanProduction Wells Violations Noted Status check inspection of this Clinton Sandstone well that was found to be over-pressurized on the 4.5 X 7 surface casing annulus and equalized with the 4.5 production casing at 490psi on 9/20/18. Enervest personnel have been selling the pressure down and the annular gauge currently indicates a pressure of 160psi and is below the maximum allowable pressure of 180psi (calculated by multiplying the depth of the casing seat (594) by .303). The chart in the meter shack (MOA) indicates that the well has cycled one (1) time over the prior 7 days and is currently at the line pressure of 160psi. Based on the equalization of pressures found between the casing and annulus in this well, it is determined that the 4.5 production casing is defective. Enervest Operating, LLC will be afforded thirty (30) days from the issuance of this Notice to perform one of the following activities; 1) repair the defective casing, or 2) plug and abandon the well. Failure to comply with this Notice may result in further enforcement action.
