Inspector Kevin Perry, Doug Gonzales, and Bishop Well Service were on location. Bishop has drilled to the 6.625 casing back-off point with a 7.875 tri-cone bit without encountering any of the clay plugs on 8/28/18. Today, they circulated the hole clean and tripped out of the hole. They plan to run in with a smaller bit and attempt to get back inside the 4.5 production casing. No violations.

The property owner has placed a galvanized culvert pipe over the well in order to vent the gas. The area is used for equipment storage by the owner dba Nowak Excavating. Mr. Nowak (330-322-6040 cell, 330-296-8392 home) was given information and applications for the orphan well program.

1293885712 2018-08-27 Orphan No Violations I was on location with Inspector Kevin Perry to observe drill-out operations on this leaking, orphan well. Bishop Well Service was currently drill out debris at a depth of 2350. They determined that the mud was not heavy enough (8.5lbs/gal.) to lift the cuttings and clean-up the hole. They pulled 10 joints of tubing and shut-down for the day. They are going to mix mud tomorrow in the A.M. No violations.
1306055234 2018-09-19 Orphan No Violations Status check inspection of operations at this orphan well. There is currently no activity on location. There is tubing set in slips on the wellhead and the well is venting natural gas and spitting up foamed water and cement every 10-12 seconds. The foam has filled the cellar and is up to the base of the working platform. The secondary containment pit is nearly full. Bishop Well services arrived on location and they are beginning to transfer fluid and foam to the 500 barrel tank on location for disposal. Plans were discussed and I left location. No violations.
1355155190 2012-12-06 Orphan No Violations I performed this inspection at the request of inspector Norburt Lowder to gather more information on this plugged and leaking, orphan well. During my inspection, I found the plugged well had an old tire around the casing/borehole with a plastic lid on top. I removed the lid and the tire and I could see remnants of the 12 PVC pipe used to vent the gas sticking out of the ground. The PVC was fit around the 10.75 casing still in the ground. The casing was 6-8 below grade and was full of water and bubbling vigorously from the escaping methane. I pulled a 7-8 piece of 4 PVC out of the water/borehole that was likely used as a vent stack on top of the 12 PVC. I took pictures and video of the borehole and the area surrounding the well. I did not use my gas detector as the smell of methane was obvious and I had previously registered an LEL reading inside the borehole of 100 on 8/1/2012. There was no visible sign that anything was leaking outside the casing. I placed the 4 PVC back in the hole and never tagged the clay plug which was noted at 2 below grade from a 1996 inspection. I originally inspected the well at the request of Nick Coia (330-957-3533) who was looking at buying the property. I tried contacting him on 11/02/2012, left a message, and have not heard back from him. A check of the Portage County Auditors records indicates that the land is still owned by Lakewood Hills, LLC who I am currently trying to find contact information. It appears that John Epling & Sue Epling are the authorized representatives for this company.

1399812422 2018-09-14 Orphan No Violations I was on location with Inspector Kevin Perry and Bishop Well Service to witness re-plugging operations at this Orphan well. The rig was unable to circulate yesterday and they are currently mixing mud (Bentonite) to get the weight up. There is an issue with the mud foaming up and Doug Gonzales arrived with five (5) gallons of defoamer to take care of the issue. Bishop began drilling the plug with a 7.875 bit and drilled from 390 to 450. They should be out of the plug at 500. No violations.
1462791907 2018-09-05 Plug / Plug Back No Violations Status check inspection of the re-plugging of this orphan well. Inspector Kevin Perry, Doug Gonzales, Bishop Well Service, and Petroset Cementing were on location performing operations. The top plug was set in two (2) stages consisting of 15.1 barrels and 37.8 barrels of cement slurry. The top plug will be checked tomorrow for integrity before Bishop rigs down. No violations.
1475895148 2018-10-03 Orphan No Violations Status check inspection of this Orphan Well operation. There is currently no activity on location. I inspected the wellhead and found 2.375 tubing in the hole and a gas cut foam at the top of the 10 casing head. The volume of gas venting from the well has been greatly diminished since blowing down the annular pressure at the Bogus #1 well (PN#20429) 2300 to the northeast. I checked the wellhead prior to leaving location and the foam level has dropped four (4) inches in two (2) hours. No violations.

1534877872 2018-08-21 Plug / Plug Back No Violations Status check inspection of this Orphan well location. My inspection found Bishop Well Services rig rigged up over the hole with a swivel and 2.375 tubing. The work area is fenced and contains two (2) open, half round circulating tanks, and a mud pump all set on secondary containment. There is a 500 barrel frac tank spotted to the south of the wellhead and the access stairs are secured with fence panels. There is currently no activity on location. No violations.
1536257713 2018-09-06 Orphan No Violations Status check inspection of the top plug set yesterday (9/5/18). My inspection found the top plug 10 below the casing head and leaking. The cement has not set up after 24 hours. There is currently no activity on location. I spoke with Inspector Bob Roush, who was on location earlier today about the plug. I checked the sample cup they collected from the top plug that was not set up inside the casing and it was found to be 90% set. The gas or an unknown contaminant has not allowed the cement in the casing to gel and harden. The plan is to clean the hole out down to hard cement and re-plug the top of the hole. No violations.

1536844116 2018-09-10 Orphan No Violations Status check inspection of re-plugging operations. Inspector Kevin Perry and Bishop Well Service were on location. The top plug was drilled out to a depth of 390 (110 below the 10 casing seat) and Bishop began tripping the bit and collars out of the hole. There was gas cut foam emanating from the annulus between the 10 casing and 2.375 tubing. Gas cut foam began flowing out of the tubing after breaking the second connection at approximately 330 below ground level. The foam continued to flow through the entire trip. The well head was plumbed back in and the hole continued to slowly unload to the steel tanks. The well will be re-evaluated tomorrow (9/11/18). No violations.

1538401484 2018-10-01 Orphan No Violations I inspected this well at the request of Orphan Well Inspector Kevin Perry to make sure the site is secure and that nothing has occurred over the past weekend. My inspection found the site secure and I found no evidence that any of the equipment has been tampered with while unoccupied by personnel. The 10 casing port is open and the 2.375 tubing is hanging from the swivel and blocks and set in the open tubing slips. I found no evidence that the well was venting gas, and I registered zero (0) LEL readings on the Ventis Pro4 gas detector above the casing head and at the port, which is open. No violations.
1538658008 2018-10-02 Orphan No Violations Status check inspection of Orphan Well operations. I was on location with contractor Doug Gonzales and Bishop Well Service (BWS). BWS tripped the tubing from the hole and took the slips off of the 10 casing. Doug had me check for gas with a VentisPro 4 gas detector. I registered intermittent Lower explosive limit (LEL) readings that maxed out at 34 confirming that there was still gas emanating from the casing. The hole was loaded with freshwater and bubbles were observed at surface, but greatly diminished from the previous weeks, prior to blowing down the back side pressure at the Bogus #1 2300 to the northeast. Kevin Perry arrived on location and BWS picked up the 9.5 tri-cone. BWS reamed the 7.875 hole inside the casing and drilled one (1) kelly of cement. It was decided to stop drilling as a precaution to not damage the surface casing. BWS is going to trip in with tubing and circulate the hole clean. No violations.

1655826392 2018-09-24 Orphan No Violations I was on location to assist Kevin Perry in the plugging of the orphan project. A downhole camera ( supplied by DOGRM ) was lowered down hole to investigate the crossflow of gas into the wellbore. Also confirmation of the casing shoe was sought. Due to turbidity ( due to gas flow ) in the wellbore fluids the camera offered minimal information. Appalachian Well Survey (AWS) was on location to determine if the bridge plug was leaking. AWS ran a temperature log which indicated that some gas could be leaking pass the bridge plug. It was decided to grout to the interval from 382 to surface utilizing approximately 7 cubic yards of nine ( 9 sack grout @ 19.4 ppg ). After running a 2.375 inch EUE tremme pipe to 378 the grout was ran into the 10 inch casing while displacing the borehole fluids out the tremme pipe. The well was filled with grout and static. While the Bishop Well Service crew was rigging down the 10 inch started to show migration of gas through the grout after 20 minutes. A natural gas indicator, supplied by Gonzoil, indicated that it was natural gas not trapped air. The well was secured overnight.
1697100227 2018-09-13 Orphan No Violations Status check inspection of re-plugging operations. Inspector Kevin Perry and Bishop Well Service were on location. Bishop had tripped back into the hole with a 9.5 tri-cone bit to drill out the 100 plug spotted yesterday. They did not encounter any cement until they reached the top of the previous drill out plug at 390. All the cement had either flowed back or been thieved by the Berea Sandstone. They attempted to drill out the rest of the previous plug but could not establish circulation. Bishop shut-down and will bring in mud to try and cake the formation in order to circulate and drill out the plug. No violations.

Bishop cleaned out the hole to the 6.625 casing back-off point at a depth of 2788 and got the mud weight up to 8.9lbs/gallon to help carry cuttings and debris. They chased something down the hole but did not encounter any clay plugs. They circulated on the hole, pulled up, and shut down for the day. The hole was making gas throughout operations. No violations.
1808595431 2012-08-01 Orphan No Violations I received a phone call from Mr. Nick Coia (330-957-3533) requesting an inspection of a leaking, plugged and abandoned well located on property he was purchasing. This well has I&O CASE # 1997 05 033. This well was plugged in 1975. The casing is just below the ground surface and the hole contains fluid and is bubbling/venting methane to the surface. The LEL on the gas detector registered 100 and methane was detected. There was an obvious odor of methane 10-15 feet from the plugged well. I talked with Terracina Maxwell and she is sending Mr. Coia information and documents for the Orphan Well Program and the Landowner Grant Program for review. The well is staked with ribbon.

1966086881 2018-09-12 Orphan No Violations Status check inspection of re-plugging operations. Inspector Kevin Perry and Bishop Well Service were on location discussing the well. I supplied a pH test strip to test the mix water in the 500 barrel tank. The water tested at a pH between 6 and 7 and was acceptable. The well is currently still gassing and unloading cement through a one-quarter open, two (2) inch ball valve. A plug was set earlier this afternoon from 390-200 with 60 sacks of Class A cement pre-mixed with 3% calcium chloride. This well will be re-evaluated tomorrow morning (9/13/18). No violations.
