0118926357 2017-10-03 UrbanProduction Wells No Violations I was on location with Fred Romeo and Terry Whitehair (DOGRM) to follow-up on Complaint #5716 and try and determine the source of the natural gas odor. Equipment was inspected with gas detectors and it was determined that the main source of fugitive gas was being emitted from the 100 barrel tank vent stacks. A call was made to Joe Teppert (Enervest) about the issue and he sent the pumper and Supervisor Josh Shrock to location to try and fix the leak. It was determined that the seating nipple in the motor valve was not fully seated to close the valve. They disassembled the top of the motor valve and fully seated the nipple. The gas venting from the tank vent stacks was fully mitigated by the repair and should satisfactorily resolve the Complaint. I met with the Complainant and explained the cause of the odor and what was done to resolve the issue. He was satisfied with the resolution and I gave him my card to contact me if the issue should arise in the future. This Complaint can be closed.
0467662934 2016-06-24 Production Wells No Violations I received a call from the Uniontown Office on 6/20/16 to contact a Mr. Cory McCray who is interested in buying the property and home where this well is located. I spoke with Mr. McCray and he was concerned with the condition of the well and asked if I could inspect the well. I inspected the well today and found the wellhead set up to produce as an open flow, swab well. The wellhead was flanged and was equipped with a master gate valve and two (2) flanged wing valves. A one inch line from the back of the wellhead was equipped with a regulator and plumbed to the flow line. The well produces through a vertical separator south of the wellhead and to a diked tank battery just south of the separator. The battery contains two (2) 100 barrel tanks with bull plugs in the lad lines and the tank access ladder is fenced. Production is being recorded at a meter house towards the back of the property. There is a disconnected domestic tap northwest of the wellhead consisting of a drip bottle, 1 line and regulator, 20 of 2 line with a regulator and ends at a disconnected drip and house meter. The proper ID is posted on the tank. Aside from cosmetic issues (peeling paint on the separator and tanks), the operation is in satisfactory condition. I found no violations at this time. I spoke with Tim McConaha (Enervest) about the domestic run and if it could be removed, and asked him to put locks on the ladder access gate and meter house. He told me he would send someone over to look at the domestic tap and lock the access gate and meter house. I spoke with Mr. McCray again and he was satisfied with my report. I will conduct a follow-up if necessary.

2147248528 2017-10-02 UrbanProduction Wells No Violations Follow-up to Complaint #5716 concerning a gas odor at this well location. I contacted the Complainant at 2:20pm and told him I was on my way to inspect the well. My inspection found the well in production and equipped with a large, flanged, open-flow wellhead. The well produces through a vertical separator and the tank battery consists of two (2) 100 barrel tanks. Production is being recorded at a metering shed (MOA) south of the tank battery. The only gas odor I encountered was at the tank battery. Only one (1) tank is plumbed to the dump line and I could see vapor emerging from the three (3) vent stacks. The tank is empty and there may be an issue with the dump line in the separator that is allowing gas to go to the tank. I spoke with Terry Whitehair (DOGRM) and he will meet with me on location in the morning (10/3/17).
