0267798062 2013-10-15 UrbanProduction Wells No Violations I inspected this well to follow up on Chiefs Order #2013-136 issued on 9/25/2013. The operator, Stephen J. Svetlak, LLC, has failed to maintain proof of liability insurance as required by ORC 1509.07(A) and has been found in material and substantial violation for failing to maintain coverage. No commercial production has been reported for this well since 1997. My inspection found a tank battery with 2-100 barrel stock tanks completely overgrown with brush and trees. The overgrowth was such that I was unable to get within 20 feet of the battery. After searching the brush to the north and east I was able to locate a horizontal separator that was in disrepair and not being utilized. I located a Dominion/East Ohio line marker and an old domestic meter in the brush. The meter did not appear to be in use. After further searching, I located 4.5 casing above grade and equipped with a 4x2 swedge and a 2 ball valve. The ball valve had a small leak and was hissing. The well head had been removed and pieces of it were lying on the ground. The disposition of the well and equipment is safety hazard due to the overgrowth and the leaking gas. This well is idle and needs to be placed back in production or properly plugged and abandoned.
1062046288 2017-05-15 UrbanProduction Wells No Violations I inspected this well location as a follow-up to Chiefs Order # 2016-171. At the time of this inspection, I observed the following: a wellhead with 4 to 2 reduction swage which had a ball valve that was closed but leaking gas. No other flow-line or supply-line appurtenances were in place. There were portions of a former wellhead valve tree and connections discarded around the wellhead. There was an associated storage tank battery located adjacent to the wellhead. The battery was comprised of two 100-barrel steel, stock tanks. The areas surrounding the well and battery were comprised of a dense overgrowth of small tress, field grasses and other wild vegetation obscuring much of the system and making access for inspection difficult. Other than the gas leak, I did not observe any other signs of impact to the surrounding areas. The access road from New Milford Road was overgrown but passable.
