0308866319 2018-09-24 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production equipment consisted of two dual sand traps, two gas production units, triethylene glycol dehydration unit, four combustors, one vapor recovery tower, and eight 400-barrel tanks. The identification was located at the wellhead. Annular pressure monitoring was not present.
0637107547 2018-01-09 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well had been completed and is now flowing back. The well was located on the Bettinger pad. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The well was flowing back through a sand trap and gas buster tank. The production equipment consisted of eight 400 barrel tanks, two gas production units, one compressor, and 4 combustors. The location meets 1509.072 (A) requirements.

1315096071 2018-10-30 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was not in place. Production equipment included an 2 individual Sand Traps, and Gas Processing Unit, as well as 8 400-barrel steel production fluid tanks, and 4 combustors that were shared with the other producing wells on the pad. There was a compressor unit and midstream equipment on the Southern Corner of the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.
1502643913 2017-08-13 Casing String Witnessed No Violations The purpose of my inspection was to witness 100 feet of the 26 inch diameter conductor being cemented in a 32 inch diameter borehole. Express Energy drilled the borehole and did not encounter any water during the drilling process. A pre-job safety meeting was held with all personnel on location including Express Energy, Antero Company Man, and BWS Trucking. Following the safety meeting Express Energy pumped 10 barrels of fresh water, 330 sacks of class A cement with 2% gel, and 55 barrels of fresh water were used for displacement. 10 feet of cement was left inside the bottom of the conductor and 18 barrels of cement returned to surface.
1505249131 2017-09-12 Drill / Deepen / Reopen No Violations During my inspection HP drilling rig 519 was rigged up and preparing to spud the surface borehole. A walk through inspection was conducted with no issues noted. 100 feet of 26 inch diameter conductor was in place and had been cemented to surface on 8/13/17. The Antero Company Man and I discussed the drilling parameters for the surface section and he plans to use a 17.5 inch diameter hammer bit drilling on fresh water with additives of Dawn soap to drill to a depth of roughly 1200 feet. Following drilling the borehole they will run 13.375 inch diameter surface casing and cement to surface. Company Man will notify me with an estimated time for the cement job.

The casing was 54.5 pounds per foot. Schlumberger Cementing was on location. The cement proposal was discussed with the Antero Company man on location. Schlumberger pumped 275 barrels of fresh water to break circulation and 30 barrels of gel. Job was partially witnessed. I was accompanied by ODNR Inspector Tom Thomas.
