0162278622 2018-09-24 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. The production equipment consisted of two dual sand traps, two gas production units, triethylene glycol dehydration unit, four combustors, one vapor recovery tower, and eight 400-barrel tanks. The identification was located at the wellhead. Annular pressure monitoring was not present.

0787013095 2017-08-09 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well was located on the Bettinger pad. During my inspection the well was equipped with a production tree style wellhead with production valves in the open position. Gauges present on the casing read 290 pounds per square inch and 280 pounds per square inch on the tubing. Annular pressure monitoring was present. Three well stakes had been driven beside the wellhead showing the proposed location of wells yet to be drilled. The production equipment consisted of eight 400 barrel tanks, one gas production unit, one compressor, and one combustor. Contractors for Antero Resources were on location setting up additional production equipment such as sand traps, gas production units, and combustors. Antero discussed the possibility of mobilizing H P Drilling Rig 519 onto the pad to drill additional wells.
0915032098 2018-10-30 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing and 2.875-inch diameter tubing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was not in place. Production equipment included an 2 individual Sand Traps, and Gas Processing Unit, as well as 8 400-barrel steel production fluid tanks, and 4 combustors that were shared with the other producing wells on the pad. There was a compressor unit and midstream equipment on the Southern Corner of the pad. There was secondary containment around the production fluid tanks. The well pad was contained within an earthen dike, there were multiple Emergency Shutoff Devices on location. Identification tags were present, and production was up to date.

1171611905 2016-06-16 Preliminary Restoration No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of eight 400 barrel tanks set in a spray lined steel containment system. The well site meets the preliminary restoration requirements of 1509.072(A).
1442500498 2015-09-17 Casing String Witnessed No Violations My inspection today finds Express Energy Services drilled a 100 of thirty inch diameter borehole. Express then ran 100 of 26 weld casing. Express Energy Services mixed and pumped 330 sacks of class A cement. I witnessed 23 barrels of cement circulate back to surface. The cement that was circulated to surface was removed from inside the cellar and placed on the back side of the cellar to seal it in place. A mouse hole with steel base plate was also cemented in place.
