0103808004 2017-06-27 Production Wells No Violations The pad is named NBL 39. There are four producing wells and two stratigraphic test wells on location. The purpose of my inspection is a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure monitoring was present on the tubing, production casing, and the annulus of the intermediate casing strings. The production equipment on location consists of one 50 barrel tank, five 400 barrel tanks, four sand traps, four gas production units, and a vapor recovery system. The production records are current and the identification is located at the wellhead. Force incorporated was offloading brine from the tank battery and TK Oil and Gas was preforming maintenance on the gas production units during inspection.
0225157024 2014-11-09 Casing String Witnessed No Violations I am onsite to witness the conductor casing installation and cementing operations. Express Energy Services drilled 100 of 36 hole and installed 100 of 30 115 pound weld casing. My pre-job mix water test, with a low range Quantab, found that the chlorides were elevated above 250 parts per million. I instructed the company to find another source of water for the job. The alternate source was tested at 96 parts per million (3.0 with a low range Quantab) and the job was allowed to resume. Express Energy Services circulated 275 sacks of class A cement to fill the annular volume of the casing. I witnessed 12 barrels of cement circulate to surface. One foot of cement was left at the base of the cellar to provide a liquid tight seal.
0248981823 2014-12-03 Casing String Witnessed No Violations I am onsite to witness the casing cementing operations of the 13.375 first intermediate casing string. The drilling rig, Savanna 640E, installed 1644 of 13.375 J-55 casing. Calfrac circulated 1335 sacks of class A cement into place. I witnessed 30 barrels of cement return to surface.
0562214967 2014-11-28 Casing String Witnessed No Violations
0648284112 2015-06-29 Completion Testing No Violations I stopped at the location to review the production casing bond log. The cement top is at 5344 according to Allied Wireline. The cement is tied into the 9.625 casing.

1572497068 2015-08-26 Preliminary Restoration No Violations I am accompanied by Inspector Selcik, our inspection today finds the well site meets the preliminary restoration requirements of 1509.072(A).
1687677486 2016-11-22 Production Wells No Violations The purpose of my inspection today is a status check on a producing well. During my inspection the well was equipped with a Cameron production tree style wellhead. The production equipment consists of five 400 barrel steel tanks, within plastic lined steel containment, and four gas production units. Production equipment is shared with the four wells on the pad 3412124517, 3412124518, 3412124523, and 3412124524. Well identification is located at the beginning of the lease road. Production has been reported for 2016. There are no violations present at this time.
1951440688 2016-09-27 Production Wells No Violations My inspection today finds the well is equipped in a manner consistent with being capable of production. The tank battery consists of five 400 barrel steel tanks set in a plastic lined steel containment system. Four production units are located near the entrance. There are a total of four wells on the pad.
1994065413 2015-05-18 Drill / Deepen / Reopen No Violations My inspection finds the drilling rig is drilling an 8.75 hole at a depth of 13190 in the horizontal section of the well.
