0470813717 2018-03-20 Production Wells No Violations The pad is named NBL 39. There are four producing wells and two stratigraphic test wells on location. The purpose of my inspection is a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure monitoring was present on all casing strings. The production equipment on location consisted of one 50 barrel tank, five 400 barrel tanks, four sand traps, four gas production units, and a vapor recovery system. The production records are current and the identification is located at the wellhead.
0515785330 2014-11-09 Casing String Witnessed No Violations I am onsite to witness the conductor casing cementing operations. Express Energy Services drilled 100 of 36 hole. Express then installed 100 of welded 115 pound 30 casing. Express also provided the cementing services by circulating 275 sacks of class A cement into place on the backside of the casing. I witnessed 10 barrels of cement returns to surface.
0534439091 2017-06-27 Production Wells No Violations The pad is named NBL 39. There are four producing wells and two stratigraphic test wells on location. The purpose of my inspection is a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure monitoring was present on the tubing, production casing, and the annulus of the intermediate casing strings. The production equipment on location consists of one 50 barrel tank, five 400 barrel tanks, four sand traps, four gas production units, and a vapor recovery system. The production records are current and the identification is located at the wellhead. Force incorporated was offloading brine from the tank battery and TK Oil and Gas was preforming maintenance on the gas production units during inspection.
0838021457 2015-05-26 Casing String Witnessed No Violations I am onsite to witness the casing string installation. The drilling rig, Patterson 801, is running 5.5 casing with centralizers.
1258532223 2015-08-26 Preliminary Restoration No Violations I am accompanied by Inspector Selcik, our inspection today finds the well site meets the preliminary restoration requirements of 1509.072(A).

1352236567 2016-11-22 Production Wells No Violations The purpose of my inspection today is a status check on a producing well. During my inspection the well was equipped with a Cameron production tree style wellhead. The production equipment consists of five 400 barrel steel tanks, within plastic lined steel containment, and four gas production units. Production equipment is shared with the four wells on the pad 3412124517, 3412124518, 3412124523, and 3412124524. Well identification is located at the beginning of the lease road. Production has been reported for 2016. There are no violations present at this time.
1449519552 2014-11-14 Casing String Witnessed No Violations I am onsite to witness the surface casing installation and cementing operations. Savanna Rig 640 ran 518 of 20 casing inside a 26 borehole. Calfrac circulated 994 sacks of class A cement into place around the backside of the casing. I witnessed good cement returns at surface.
1812799451 2015-05-11 Casing String Witnessed No Violations My inspection today finds the drilling contractor, Patterson Drilling Rig 801, is running 5.5 casing at a depth of 5000. Centralizers are being placed on every joint in the lateral section of the well.
