0573544650 2015-11-10 Completion Testing No Violations I received an email from Ian Leslie, Consol Completions Superintendent, indicating that at approximately 02:00 a 200 barrel release of chlorine dioxide and scale inhibitor treated water was released on the NBL 39 Pad. I arrived on site at 08:37. Upon arrival, I met with Brian Becker, Consol Water Foreman. Mr. Becker gave me a description of the events. A six inch cam lock hose was disconnected from the distribution manifold by a Rock Water employee to remove a rock from the valve, before the hose was pulled out of the full 500 barrel frac tank, causing the water to siphon out of the frac tank and onto a small containment. A total of four fresh water tanks were connected to each other. A prerelease tank measurement and a post release measurement were taken to quantify the release. The rain water sumps were closed as soon as the release was reported. 150 barrels have been recovered so far, with an additional Clear Water vac truck in route. All water recovered will be utilized in the frac operations. I found no evidence of any water leaving location. According to Brian Becker the water had not yet been treated with scale inhibitor when the release occurred. A test of water standing onsite with a low range Quantab indicated chlorides are at a concentration of 115 ppm. The site has received a light rain during the entire time of the clean-up.
0805974632 2014-09-09 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Keane Drilling rig 2144 drilled 586 of 26 hole and then ran 20 casing to a depth of 520. Allied Oil and Gas Services cemented the casing in place with 1050 sacks of class A cement with 2% CaCl and .25 pounds per gallon LCM. I witnessed 43 barrels of cement returns to surface.

The purpose of my inspection is a status check on a production well. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Pressure monitoring was present on the tubing, production casing, and the annulus of the intermediate casing strings. The production equipment on location consists of one 50 barrel tank, five 400 barrel tanks, four sand traps, four gas production units, and a vapor recovery system. The production records are current and the identification is located at the wellhead. Force incorporated was offloading brine from the tank battery and TK Oil and Gas was preforming maintenance on the gas production units during inspection.

During my inspection the well was equipped with a Cameron production tree style wellhead. The production equipment consists of five 400 barrel steel tanks, within plastic lined steel containment, and four gas production units. Production equipment is shared with the four wells on the pad 3412124517, 3412124518, 3412124523, and 3412124524. Well identification is located at the beginning of the lease road. Production has been reported for 2016. There are no violations present at this time.
