0425471861 2014-08-30 Casing String Witnessed No Violations I am onsite to witness the surface casing cementing operations. Savanna Drilling rig 640E drilled a 26 hole to a depth of 427 then ran 20 casing to a depth of 427. Calfrac cemented the casing in place with 161 barrels of class A cement.

1064954700 2017-01-23 Production Wells No Violations The purpose of my inspection today is a status check on a producing well. There are 3 wells on this pad and the pad is named NBL 36. During my inspection the well is equipped with a production tree style wellhead. The production equipment for this well consists of nine 400 barrel tanks within containment, three gas processing units, one vertical heater treater, and one fire tube reboiler. The vapor recovery system consists of one horizontal separator and one combustor. The production records are current, the identification is located by the wellhead, and there are no violations present.
1086621458 2018-11-28 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter casing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, the 13.375-inch diameter casing had 100 pounds per square inch, the 9.625-inch diameter casing gauge read 0 pounds per square inch. Production equipment included a sand trap, gas processing unit, as well as 10- 400-barrel steel production fluid storage tanks, and a combustor shared with the other wells on the pad. There was secondary containment around the production fluid tanks. The well pad was had earthen containment surrounding it, there were multiple Emergency Shutoff Devices around location. There was a midstream operation on the southeast corner of the well pad. Identification tags were present, and production was reported for 2018.

1410311021 2014-09-09 Casing String Witnessed No Violations I am onsite to witness the witness the second intermediate casing cementing operations. The drilling rig ran 7245 of 9.625 casing. Calfrac cemented the casing in place with 331 barrels (770 sacks) of light weight cement and 67 barrels (307 sacks) of class H tail cement. All of a dyed spacer was returned to surface indicating proper cement placement.
1873228477 2018-09-20 Production Wells No Violations The purpose of my inspection today is a status check. During my inspection the well is equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of three gas production units, one combustor, and nine 400-barrel tanks. The identification stated CNX Gas Company. Ascent will have 60 days after the transfer date to post identification.
