0532796605 2015-08-26 Preliminary Restoration No Violations I am accompanied by Inspector Selcik, our inspection today finds the well site meets the preliminary restoration requirements of 1509.072(A).
0563746263 2018-11-28 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter casing that appeared to be open flowing. At the time of my inspection annular pressure monitoring was in place, the 13.375-inch diameter casing had 100 pounds per square inch, the 9.625-inch diameter casing gauge read 200 pounds per square inch. Production equipment included a sand trap, gas processing unit, as well as 10- 400-barrel steel production fluid storage tanks, and a combustor shared with the other wells on the pad. There was secondary containment around the production fluid tanks. The well pad was had earthen containment surrounding it, there were multiple Emergency Shutoff Devices around location. There was a midstream operation on the southeast corner of the well pad. Identification tags were present, and production was reported for 2018.
0565462441 2017-01-23 Production Wells No Violations The purpose of my inspection today is a status check on a producing well. There are 3 wells on this pad and the pad is named NBL 36. During my inspection the well is equipped with a production tree style wellhead. The production equipment for this well consists of nine 400 barrel tanks within containment, three gas processing units, one vertical heater treater, and one fire tube reboiler. The vapor recovery system consists of one horizontal separator and one combustor. The production records are current, the identification is located by the wellhead, and there are no violations present.
0801409118 2016-04-14 Production Wells No Violations My inspection today finds the well site has been cleaned and all contaminated soils removed. A test of standing water, in the area previously impacted, with a high range Quantab failed to find measureable chlorides present. The wellsite meets the preliminary restoration requirements of 1509.072(A).
1095398499 2014-08-22 Casing String Witnessed No Violations I am onsite to witness the cementing operations of the second intermediate casing string. Savanna 640e ran 1678 of 13.375 casing. Calfrac cemented the casing in place with 1336 sacks of class A cement with .125 pounds per sack celloflake and 1% CaCl.
1105085235 2014-08-08 Casing String Witnessed No Violations I am onsite to witness the cementing operations of the surface casing string. Circulation could not be gained by the drilling rig. The decision was made to spot a 64 barrel balance plug at the base of the casing. 1.25 tubing was ran to a depth of 296 and 220 barrels of cement were circulated in place. The tubing was pulled and the fluid level was found at 160 from surface. I instructed the company to wait eight hours and the run the tubing back in and gain circulation before cementing to surface.

1378984440 2014-08-18 Drill / Deepen / Reopen No Violations I am onsite to witness the casing integrity test. Several attempts were made before a problem with the pressure recording chart drive could be fixed. The casing lost less than ten percent of total pressure.

1459876599 2016-04-05 Production Wells No Violations At 10:33 AM I received a telephone call from CNX production manager Cody Craker indicating that an approximately two barrel release occurred as a result of a washed out valve on between the sand trap and the production unit. Upon my arrival onsite I witnessed seven Sunpro personnel digging sumps and placing absorbent booms and clothes. I tested a small puddle 52 from the source of the release with a high range Quantab, and found chlorides in excess of 6600 parts per million. Sunpro immediately removed the standing fluids with a vacuum truck. I instructed the company to keep all drainage sumps closed until the release is cleaned.
1774601926 2018-09-20 Production Wells No Violations The purpose of my inspection today is a status check. During my inspection the well is equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of three gas production units, one combustor, and nine 400-barrel tanks. The identification stated CNX Gas Company. Ascent will have 60 days after the transfer date to post identification.
