0209662355 2019-03-21 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 5.5-inch diameter production casing 2.875-inch diameter tubing utilizing a plunger lift system as means of production. Annular pressure monitoring was in place on all producing wells on the pad. Production equipment included a Sand Trap, a gas processing unit and six 500-barrel production fluid storage tanks that were shared with the other producing wells on the pad, there was secondary containment around the production fluid storage tanks. There were multiple Emergency Shut Down devices on location, Identification tags were present and had been updated to Ascent Resources and production was reported for 2018. There was a midstream valve site on the south side of the pad. At the time of my inspection I noticed a slip on the North side of the well pad and informed the operator of the problem as well as contacted ODNR Engineering Tech Andrew Thomas to inform him of the problem.
0379291931 2015-01-13 Preliminary Restoration No Violations My inspection today finds the well site meets preliminary restoration requirements. The tank battery consists of six 500 barrel tanks set in a liquid tight containment. Contractors are onsite building the production infrastructure.
0608670381 2017-08-24 Production Wells No Violations The purpose of my inspection was a status check on a production well. The well was equipped with a Cameron production tree style wellhead. Production valves were in the open position. A gauge reads 0 pounds per square inch on the annulus between the 5.5 inch diameter production casing and the 9.625 inch diameter intermediate casing. A gauge reads 0 pounds per square inch on the annulus between the 9.625 inch diameter intermediate casing and the 13.375 inch diameter intermediate casing. The production equipment consisted of six 500 barrel tanks within steel line containment. A vapor recovery system to capture tank vapors consisted of one vent scrubber and one combustor. Three gas production units, one vertical heater treater, and one indirect line heater were located on the west side of the pad. The identification was located at the wellhead.

Ascent will have 60 days after the transfer date to post identification.

There are no UIC numbers on the tank of the truck. I informed the drivers of trucks 3281 and 5373 that they were not in compliance with UIC regulations, and told them get this corrected before hauling any production fluids.
1391607989 2014-02-02 Plug / Plug Back No Violations My inspection finds the Savanna Drilling, 640e, ran open ended drill pipe to a depth of 8262. Calfrac spotted a 313 sk type H cement plug with a .93 yield and a weight of 17.5 pounds from 8262 to 7737. The rig pulled the drill pipe up to 7737 and circulated hole volume. Calfrac then spotted a 204 sk 50/50 poz. mix plug up to 7147.
1442008590 2017-03-09 Production Wells No Violations The purpose of my inspection was a status check. TK Oilfield Service was on location removing a Frac tank and containment liner from the pad. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges present read 590 pounds per square inch on the casing and 310 pounds per square inch on the tubing. Annular pressure monitoring was present. The production equipment consists of four gas production units, six 500 barrel tanks, and one combustor. Identification was present and production records were current.

Mr. Force asked for 30 days to get all of his trucks properly identified to 1509:223(D) specifications. I told him that the Division would give him until Feb 16, 2014 to comply or he could face enforcement action such as, but not limited to, an order to cease operations.
1599649870 2014-01-27 Drill / Deepen / Reopen No Violations My inspection today finds the drilling rig is drilling out the cement at the base of the 9.625 casing. They are drilling a 8.75 pilot hole.
1809264072 2014-01-13 Casing String Witnessed No Violations I am onsite to witness cementing operations. Savanna rig 640E ran 2003 of 13.375 casing. Calfrac cemented the casing in place with 1429 sks of class A cement. I witnessed 19 bbl of cement returns to surface.
