0568775117 2009-05-07 Production Wells No Violations Inspection this day found well not in production. Well has jack, motor, no belts, weeds growing high around it tells me no one has tended well in some time. Small 50 bbl plastic tank nearby, no IDs. Valves shut. Well needs NOV.
0590646963 2016-03-16 Administrative Inspection No Violations I received a written correspondence from Ben Smeltzer indicating that the company was trying to determine if the well could be returned to production. The letter included a plan to plug the well if needed; however, there was no timeline included. I emailed Mr. Smeltzer and requested a timeline for the work.
1021175334 2017-09-20 Plug / Plug Back No Violations The purpose of my inspection was a status check on plugging operations. The 2 inch diameter tubing was tripped out of the hole and the bottom of surface plug top was tagged at 352 feet in depth. The tubing was tripped back in hole to 100 feet and a 100 foot plug of 24 sacks was pumped. A cement sample was pulled and weighed in ODNR assigned mud scales and the cement weighed 15.5 pounds per gallon. The tubing was tripped from hole and 9 sacks of cement were used to top the well off and bring cement to surface.
1276877234 2016-09-27 Production Wells No Violations My inspection today finds the well is equipped with a pumping unit. The unit has several trees growing up inside of it. All production valves are closed at the wellhead. A fifty barrel plastic tank is set twenty feet from the wellhead.
1478613938 2016-11-03 Production Wells No Violations ODNR Division of Oil and Gas supervisors Dan Goins and Eric Conrad, and I met with Triad Hunter representatives Austin Clark, Mike Horan, and Bob Handschumaker. We discussed the violation issues with this well and a timeline in which these violations shall be put back within compliance. Triad stated that they plan to plug this well.
1505922155 2017-09-19 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging operations. The well was equipped with 454 feet of 8.625 inch diameter surface casing and 7.875 inch diameter borehole from 454 feet to 820 feet. Patchwork Oil and Gas is the contracted cementing company for Triad. During my inspection Patchwork Oil and Gas had their service rig and water truck on location. Patchwork batched mixed the cement on location by using 2 tubs with the capacity of 10 mixed sacks of cement each. Both tubs were filled with water and 10 bags of class A Portland cement were dumped into each tub. After mixing the two tubs a pull start pump with a gasoline motor was utilized to pump the cement downhole. This method was repeated until 60 sacks total of class A Portland cement was pumped down hole to make up the bottom hole plug. The plug was pumped through 2 inch diameter tubing at 810 feet in depth. 1.8 barrels of fresh water were pumped for displacement. Patchwork tripped the 2 inch diameter tubing out of hole and ran back in the well to tag the top of the bottom hole plug. The top of the plug was tagged at 608 feet. The 2 inch diameter tubing was tripped back in hole to 354 feet and a 200 foot plug consisting of 58 sacks of class A Portland cement were pumped to make up the bottom of the surface casing plug from 354 feet to 545 feet.
1507202471 2017-10-04 Final Restoration No Violations The purpose of my inspection was a final restoration status check. During my inspection the lease road and well location had been restored. No debris from plugging operations had been left behind. This location has met 1509.072 B requirements.
