0000PL7269 Preliminary Restoration No Violations Preliminary Restoration done information from Data Point
0189155725 2013-12-31 Plug / Plug Back No Violations Chipco is running pipe down to the second plug to drill it out. They encountered the plug at 1290. The cement is very hard.
0282490621 2014-01-03 Plug / Plug Back No Violations The well had 300 psi and the fluid level was at surface. Chipco finished drilling out the bottom of surface plug. No increased flow was encountered under the plug.
0284950177 2013-11-25 Plug / Plug Back No Violations My inspection today finds Chipco rig 6 is onsite to plug the well. The well is known to have H2S. Enervest treated the well with sulfur clear a month ago. The well has 850 psi on the casing. I instructed the rig crew to set up a flare line and burn the gas off.
0464875986 2013-12-03 Plug / Plug Back No Violations Chipco is installing a hot tap on the 8.625 pipe. The pipe has 1200 psi. After installing the tap they flared the gas down.
0554376480 2013-12-20 Plug / Plug Back No Violations I met two Chipco hands at the location. The well had 700 psi on the surface casing. The pressure was vented into a 300 bbl tank on site.
0612864742 2013-12-04 Plug / Plug Back No Violations My inspection today finds the well is static. Chipco ripped the casing at 3357 and set a 45 sk plug at that point. They pulled up to 1500 and set a 60 sk plug. The surface plug was placed from 200 to surface with a 60 sk plug. The cement stayed at surface.
0681377718 2014-01-02 Plug / Plug Back No Violations My inspection today finds the well had 550 psi this morning. Chipco drilled down to approximately 1400. They cleaned the hole and picked the pipe off bottom before shutting the well in.
0853162104 2014-01-15 Plug / Plug Back No Violations I received a call from Chip Smith of Chipco at 9:21 am. Mr. Smith told me that the well had 800 psi. on it this morning. I spoke to Tim McConaha with Enervest we agreed that Enervest is to provide a weekly monitoring of the well until a plan to replug the well can be made.
0967710410 2014-01-10 Plug / Plug Back No Violations My inspection today finds the well has 100 psi on it. The fluid level is at surface. Chipco checked the plug that was set the previous day. They found the plug top at 2400. The rig crew pulled the tubing up to 1590 and set a 150 sk 500 plug and pulled the tubing out of the hole. The well was then shut in.
0983288873 2016-09-20 Final Restoration No Violations My inspection today finds the final restoration is complete. The well site is covered in vegetation and no erosion was noted. The well casing has been cut off.
1085912891 2014-01-13 Plug / Plug Back No Violations My inspection today finds the well has 40 psi on it. The rig crew ran the sand line to 984 and tagged the top of the plug. 2.875 tubing was ran to 984 and a 150 sk plug was circulated in place. The tubing was removed and the well was shut in.
1181717371 2016-04-14 Production Wells No Violations My inspection today finds the well has no pressure on it and no gas flow can be detected. Enervest has monitored the well since plugging and no gas has been detected since shortly after plugging the well. For these reasons, I have determined that Enervest can proceed with the final restoration of the well.
1225743193 2013-12-26 Plug / Plug Back No Violations My inspection today finds Chipco is setting up the blow out preventer. And preparing to drill out the top plugs.
1251756982 2013-12-26 Plug / Plug Back No Violations On site with local inspector to discuss plugging plan for said well, it appears that they have gas migration to surface and decision was made to drill out plugs to previous rip point and replug well from that point.
1386010172 2013-11-29 Plug / Plug Back No Violations Appalachian Well Service perforated the big lime at 3977 with ten shots in ten feet.
1386081189 2013-12-02 Plug / Plug Back No Violations Chipco set a 30 sk plug at 5435, and a 60 sk plug at 3977, 50/50 cement. Good circulation.
1388319051 2013-12-28 Plug / Plug Back No Violations On site to witness proper well control during cleanout process of leaking well, they encountered gas flow@110 and we let gas pressure bleed off into 300 bbl tank and once pressure was maintained they resumed drill out procedure and we reached a depth of 512 today and will resume operations on Monday 12/30/13.
1388787411 2014-01-03 Plug / Plug Back No Violations Inspection revealed that service rig tagged rip plug@2990 and they drilled 10 of it to confirm that it was good cement then decision was made to clean up hole and then come out of hole with clean out string and prepare to run tubing back into well and replug it next week.
1389278318 2014-01-09 Plug / Plug Back No Violations On site to witness plugging operation,Chipco pumped 187 bbl of 10# brine in hole and well was static and then they pumped 500 Class A cement plug and then pulled 1500 of tubing and then packed wellhead off and will check and see if there is any pressure present in the morning if no pressure present they will finish plugging said well.
1406812784 2014-07-31 Plug / Plug Back No Violations My inspection today finds the location has been reclaimed. The silt fence is still in place and vegetation is beginning to grow.
1413807916 2013-11-26 Plug / Plug Back No Violations My inspection today finds the well has 450 psi. Chipco staged a 300 bbl tank on site and flowed the well into it. The pressure fell quickly and the well continued to flow a little. The rig crew began to pull rods. No H2S has yet been detected.
1615081329 2012-10-24 Production Wells No Violations My inspection today finds the well is shut in. The well is equipped with 4.5 casing, 2.375 tubing, rods, and a pump jack. The tank battery consists of one 210 bbl tank and a separator contained in a dike. The identification is proper. The tank has multiple leaks from 5 up. There is a trail of oil that has run down the side of the tank. A small pool of oil is present at the base of the tank. I contacted Tim with Enervest and instructed him to empty the tank.

The decision was made to set a solid plug from the top of the last pug to surface. Chipco set a plug to surface. A very slight gas flow was detected so they pressured up to 240 psi to compress the cement and prevent cutting.
