0128844482 2018-07-18 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 4.5-inch diameter production casing, a 4.5-inch by 2-inch diameter swage nipple, and a valve in the closed position. There were ten 500-barrel Frac tanks on location. I called Corey McCudden with Northwood Energy to ask him what the plans were for the well. Mr. McCudden stated that on 7/31/18 they plan to re-stimulate the Ohio Shale. The will remain within the same pay zone as prior production so no permit will be required to preform this work.

During my inspection the well was equipped with 4.5 inch diameter production casing with a tubing head, 1.5 inch diameter tubing, rods, and a pumping unit powered. The production equipment consisted of one 100 barrel tank and one vertical separator. The production records were current.

1340711881 2012-06-26 Production Wells No Violations Inspection of the well head found the following equipment , 4.5 production casing , 1.5 tubing & rods with the production valves open and the well being produced by the means of a pumping unit . A single 100 barrel tank and separator are present with proper well identification . Inspector will note that the tank battery will not meet SPCC rules.
1413847295 2018-11-08 Production Wells No Violations On the day of my inspection I witnessed production equipment included a 100-barrel production fluid tank, and vertical separator that was laying on its side. The well was equipped with 4.5-inch diameter production casing. R&B Drilling Service rig was currently on location and preparing to run 2.375 inch diameter tubing and rods in the well. After speaking with Service Rig Operator Derrick Buxton, he informed me that they had drilled the frack balls and plugs out of the well on 11/7/2018. The well was not hooked up to production equipment at the time of my inspection. At the time of my inspection there were twenty 500-barrel horizontal portable fluid storage tanks, and a 300-barrel horizontal portable fluid storage tank on the location. There was also a Brad Foote pumping unit with an electric motor just off the location. There were no Identification tags present on the well head or production equipment, production was reported for 2017.
1531756561 2018-07-16 Completion Testing No Violations Status check inspection found the well production tubing and rods have been removed from the well and the 4.5diameter production casing has been reported to of been logged and perforated up hole. Crews from Corp Oil Field service is moving in 300 barrel frack tanks preparing to stimulate the well up hole. No permit to recomplete was found in the database and the lead ODNR county inspector Austin Guthridge was notified along with Patty Nicholas in the Divisions permitting section.
1540392041 2018-10-24 Production Wells No Violations On the day of my inspection I witnessed production equipment included a 100-barrel production fluid tank, and vertical separator. The well was equipped with 4.5-inch diameter production casing, 4.5-inch diameter to 2-inch diameter swedge with a 2-inch diameter Tee. The 2-inch diameter Tee had a bull plug and .25-inch needle valve from the top and a 2-inch diameter valve off the side that was in the closed position. The well was not hooked up to production equipment at the time of my inspection and after speaking with ODNR inspector Austin Gutridge, Northwood Energy Corp recently recompleted the upper section of the Ohio shale. At the time of my inspection there were twenty 500-barrel horizontal portable fluid storage tanks, and a 300-barrel horizontal portable fluid storage tank on the location. There was also a Brad Foote pumping unit with an electric motor just off the location. There were no Identification tags present on the well head or production equipment, production was reported for 2017.
